Vertigo is a common clinical symptom, and its mechanism of occurrence is complex. In addition to medical and ENT disorders, it is also related to the occurrence of cervical spondylosis. The vertigo caused by factors related to cervical spondylosis is called cervical vertigo, which can occur in both sympathetic cervical spondylosis and vertebral artery type cervical spondylosis. (1) Prone position tendon manipulation: take the patient in the prone position, use the one-finger Zen pushing, rolling and kneading method to routinely operate on the neck, shoulders and upper back for about 5 min; then use the abdominal force of the index, middle and ring fingers to push along a straight line from bottom to top, with both hands cooperating and alternating, including the neck section of the Governor’s Vessel and both sides of the bladder meridian, 3-5 times for each line; then use the abdominal force of the middle finger to push along a straight line with moderate pressure. Then, using the middle finger, push along the neck ligament and its two sides from bottom to top with medium strength, alternating between the two hands, repeating 3-5 times; finally, in the occipital and cervical area, make deeper and heavier hooking and plucking points (muscle spasm and joint capsule pressure pain points) for 0.5 min each, in order to produce an obvious local warmth. Press and rub the points of Baihui, Fengchi, Tianzhu and Tianzong, each point for 0.5 min, as strong as the patient can tolerate. (2) Supine stretching method: The patient lies on his back and the doctor stands at the head side of the bed. Let the patient’s head rest against the abdomen of the doctor, maintain a neutral position, jaw inward, neck slightly forward flexion, an assistant to fix the patient’s ankles. The doctor’s palm root of one hand holds the patient’s occiput and upper cervical segment, and the thumb and remaining fingers clamp the posterior transverse process of the affected vertebral body (based on x-ray and palpation positive points; if the misalignment is multi-segmental, the lower cervical spine is treated first, followed by the upper cervical spine), the other hand is fixed in the jaw, the doctor’s body is tilted back, driving both hands to coordinate the force of longitudinal traction, so that the point of force is located between the spinous processes of the affected vertebrae, for 5-10s, when the hands feel deep Cervical muscle tightening, the affected vertebral space will be pulled open, suddenly and smoothly increase the force of super-limit pull, at this time can hear the sound of joint or feel the sense of misalignment between the vertebrae under the hands, the effect is better. The operation can be repeated 3-4 times. The force and amplitude of pulling should be as much as the patient can tolerate. After the operation, the patient has a clear sense of relaxation in the cervical-occipital area, and even a feeling of unrest in the entire spine. No rotation or wrenching is done during the operation. The treatment is given once every other day, 10 times as a course of treatment. 1.2, warm gallbladder soup oral composition of the formula: 15 g of half summer, 9 g of Chen Pi, 15 g of Poria, 6 g of licorice, 10 g of Citrus aurantium, 9 g of bamboo rhizome, 6 g of Huang Lian, 12 g of Ze Di, 12 g of Atractylodes macrocephala, 3 slices of ginger, 3 red dates. To relieve pain. Take 1 dose of decoction daily for 20 d as 1 course of treatment. Cervical vertigo refers to vertigo caused by organic or functional changes in the cervical spine and its related soft tissues (joint capsule, ligaments, nerves, blood vessels, muscles, intervertebral discs, etc.). The pathogenesis of vertigo is complex, but it can be summarized into three aspects, (1) cervical spine bony lesions. These include cervical spine osteophytes, cervical disc herniation, cervical spinal stenosis and cervical spine developmental abnormalities and deformities, which stimulate or compress the vertebral artery, resulting in spasm or stenosis of the vertebral artery and slowing down the blood flow of the vertebrobasilar artery, leading to ischemia and vertigo. (2) Soft tissue lesions around the vertebral artery, such as long-term tissue lesions forming scar adhesions to irritate or compress the vertebral artery and its sympathetic nerves, resulting in insufficient blood supply. (3) Changes in the rheology of blood flow, mainly referring to the vertebrobasilar artery itself due to sclerosis or developmental abnormalities that lead to narrowing of the lumen and loss of elasticity of the canal wall, resulting in a decrease in the blood flow of the vertebral artery and causing insufficient blood supply. In terms of treatment, some scholars believe that correction of cervical flexion is an important part of the treatment of this disease; others believe that strong stimulation at pressure points relieves soft tissue pain and eliminates and relieves symptoms. We believe that the above factors are interrelated and affect each other, and should be taken into account in treatment. The essence of supine tendon manipulation includes two steps: supine tendon manipulation and supine stretching manipulation. On the one hand, the supine tendon manipulation directly acts on the pressure and pain points or some positive reaction points in the neck, which can quickly and effectively relieve muscle spasm, relieve spasm and pain, and eliminate the pathological reaction phenomena such as “tendon knot” and “tendon gathering”, meanwhile, the plucking tendon manipulation makes At the same time, the stretching and tendon manipulation can relax the soft tissues and improve blood circulation, which is conducive to the restoration of power balance and local inflammation, and can reduce the compression and stimulation of blood vessels and nerves due to muscle spasm, thus improving the blood supply to the basilar artery and relieving the symptoms of cervical vertigo. Moreover, it also prepares for the extraction and stretching method. On the other hand, the supine stretching and pulling method generates tension elasticity through the pulling of the joint capsule of the ligament around the joint by the force of manipulation, which readjusts the joint position and corrects or partially corrects the local environment such as joint misalignment, interlocking and synovial inlay, thus playing a role in improving the dizziness. Thus, it can be seen that the prone tendon pulling and stretching method can improve the clinical symptoms by changing the local environment of the cervical spine and its surrounding tissues. Cervical dizziness belongs to the category of “bone paralysis”, “blood paralysis”, “Xiangqiang” and “vertigo” in Chinese medicine. It is mostly caused by wind, cold, dampness, trauma, strain, etc., which leads to sinew out of groove and bone misalignment, and eventually causes qi and blood fetish obstruction, sinews and veins not honored, and clear orifices not nourished, resulting in neck pain and vertigo. For the treatment of this disease, the basic treatment principle is to activate the blood to dispel the fetish and relax the tendons and activate the collaterals, and the manipulation treatment has certain advantages in this regard, such as “Nei Jing? It is pointed out in the “Treatise on the true evil of the separation” that “snap and follow, cut and disperse, press and stop”, and in the “Golden Guide to Medicine? Orthopaedic Heart Method Essentials” cloud: “bone seams open wrong, blood fatigue, for swelling for pain, it is appropriate to use massage method. Press its meridians, can pass the depressed closed qi, and its kuiju, to disperse the fetish of the swelling, its suffering can be healed”. The treatment of manual methods to achieve the opening of the meridians, correction of its misalignment or disorder, the smooth flow of Qi and blood, then dizziness, headaches and other symptoms and healing. In our opinion, cervical vertigo is mainly caused by insufficient medulla oblongata, obstruction by phlegm and stagnation, and phlegm disturbing the clear orifices due to disorder of lifting and lowering. In clinical cases, phlegm and turbidity are blocked or heat is turned into heat, clear yang does not ascend, turbid yin does not descend, and clear orifices are obscured. Therefore, the symptoms include dizziness of the head and eyes, rotation of vision and even collapse, accompanied by nausea, vomiting of phlegm and saliva, chest tightness and poor appetite. Therefore, warm gall bladder soup is used as the main treatment with added flavor. In this formula, Er Chen Tang is added with Atractylodes Macrocephala to dry dampness and dissolve phlegm, strengthen the spleen and harmonize the stomach. The whole formula is effective in drying dampness and dispelling phlegm, strengthening the spleen and stomach, clearing the middle and resolving turbidity, so that the vertigo will be removed if the turbidity rises and falls. 3.Advantages The pathogenesis of cervical vertigo is complicated, and the treatment is also difficult. In clinical practice, comprehensive treatment is often required, and TCM has great advantages in this regard. This method is the author’s clinical summary for many years. The advantage of this method lies in the combination of the manipulation (prone stretching and pulling tendon manipulation) and the oral administration of Chinese herbal medicine, which is based on the holistic view of Chinese medicine, so that the local treatment and overall conditioning can be balanced and coordinated with each other, and the vertigo and other symptoms can be relieved. Compared with other methods, the tendon manipulation can solve the pathological factors such as bone and soft tissue, while the oral administration of Chinese medicine improves the sclerosis and blood flow of vertebral artery, thus achieving good results.
