Common prevention and treatment strategies for traumatic scarring In disasters, we most often see large wounds. Studies have shown that scarring is the result of trauma repair and is inseparable from trauma repair, and the relationship between scarring and tissue damage repair is the relationship between the “symptoms” and the “essence”.  The formation of traumatic scar and the repair of traumatic tissues is an inseparable process, and the repair of traumatic tissues starts in the post-traumatic decomposition and metabolism period, and the formation of traumatic scar also starts.  The repair and healing process of closed trauma can be divided into 3 stages: inflammatory reaction period, fibrous tissue formation period and scar formation period. After the wound of open trauma is cleared and sutured, all levels of tissues of the wound are well aligned and can heal without infection, which is called phase I healing. However, due to heavy contamination and tissue destruction, stage I suturing is not possible. In the open case, the granulation tissue keeps proliferating to reach healing, and in this case, the inflammatory reaction period and the fibrous tissue formation period are prolonged, and a particularly large amount of scar tissue is formed, which cannot disappear despite long-term shaping. Thus, the prevention of traumatic scarring should be started right after the trauma.  The prevention of scar formation period mainly focuses on taking effective measures for the scar proliferation process before the scar matures after the wound heals, and the main methods include compression therapy, silicone therapy, drug therapy, radiotherapy, physical rehabilitation comprehensive therapy, etc. The purpose is to slow down the proliferation of scar and pass through the proliferation period and enter the maturation period, transforming into mature scar or close to normal skin tissue.  At present, there are many methods to prevent scar proliferation, but the effect is still limited. Most of the methods are combined with 2-3 methods that are easy to use and have less side effects according to the scar site and characteristics, and the effect is better than a single method. In recent years, the use of Co2 fractional laser for scar prevention is gaining attention and is expected to become an important method for preventing scar growth.  Common prevention and treatment strategies for repairing burn scars 1, large area burns are also common after disasters, and from the rescue situation, early surgery is required for burns in functional areas, and the integration of plastic and orthopedic surgery into burn treatment can avoid and reduce scar growth that affects appearance and function.  2.Resume functional exercise as early as possible, massage and passive and active exercise in a timely manner after the trauma heals until full functional recovery. On the basis of exercise, use pressure bandages during sleep or rest to maintain the healing effect.  3.Warm bath during the recovery period: Bathing in warm water helps to promote blood circulation on the body surface, soften scars and enhance vitality.  4.Avoid factors that stimulate scar proliferation or pigmentation, such as sunlight exposure and use of cosmetics.  5.Rational diet: avoid drinking a lot of alcohol, eating chili and garlic and other stimulating foods after damage; eat less food containing pigment; you should eat more fresh green leafy vegetables, fruits, eggs, lean meat, milk and other foods containing vitamin C, E, AD and essential amino acids to improve the metabolic function of epidermal cells, make skin cells delicate and lubricated, and reduce scar hyperplasia.  Once a scar has formed due to too severe an injury or neglected prevention, there are also a variety of effective methods to help improve appearance and restore function.  1.Use of adhesive dressings that inhibit the proliferation of fibrous tissues and metabolism of pigment cells: softening and flattening of scar tissues and fading or disappearing of pigment; such as silicone iron.  2.Elastic compression: tissue ischemia and hypoxia under elastic compression, thus hindering cell metabolism, reducing fibroblast proliferation and limiting scar formation, such as elastic bandage and elastic compression sleeve.  3.Medication: Application of hormonal drugs, using their anti-inflammatory, reducing blood supply and inhibiting fibroblast proliferation and prompting fibroblast degeneration to inhibit scar proliferation, such as conning ketone.  4.Plastic surgery and cosmetic treatment: apply plastic surgery to remove or slacken scars to improve appearance or restore function; apply cosmetic skin care techniques to cover up and beautify defects after injury.  Patients will have hyperpigmentation six months after the healing of burn wounds, which starts as dots and then fuses into brown patches. Pigmentation can take the following measures to make it recede: 1, the skin is relatively tender after burns, extremely sensitive to sunlight, avoid direct exposure to ultraviolet light (sunlight), wear a straw hat or long-sleeved clothes when going out to cover the skin.  2.Massage the pigmented area with your hand twice a day.    3.Take vitamin C orally, 300mg per day. 4.Apply scarping ointment, 2~3 times per day.  5.Apply scar patch to the wounded area.  Care of scar proliferation period Prevention of proliferative scars after deep burns should be started within 3 months after wound healing.  1.The healed skin should be frequently bathed with warm water (water temperature 38~39℃), and the scars should be gently wiped with a soft towel coated with neutral soap to remove the skin flakes, but care should be taken not to damage the epidermis.  2.Apply scarping ointment to keep the scars moist and inhibit the growth of scars.  3.Soak the scar skin with traditional Chinese medicine Wu Jia Pi soup once a day to reduce edema and promote lymphatic venous return.  4.Wrap the elastic bandage. Pay attention to avoid pulling and rubbing the scar to avoid blistering and rupture.  Accidental disasters are very common in life, especially fires, which cause extensive disfigurement to patients, so patients and friends are more concerned about the treatment methods, hoping to find a better treatment method.
