The uterus is located in the middle of the pelvis and is the organ that produces menstruation and nurtures the fetus. The uterus is held in place by the round, broad, sacral and main ligaments, and the position of the uterus is related to the length and stretch of each ligament. According to the relationship between the uterus and the longitudinal axis of the pelvis, it is divided into anterior, posterior and horizontal positions. The anterior position of the uterus is the most common, while the posterior and horizontal positions are relatively rare. However, the posterior position of the uterus is not a disease, but a result of the long round ligament and the posterior position of the broad ligament. The environment of the uterine cavity and the state of the endometrium are not different from other positions of the uterus. As long as it is not severely posteriorly tilted and retroflexed, it does not have any adverse effect on pregnancy. Only at the end of pregnancy the enlarged uterus may press on the lumbosacral region causing lumbar soreness, which may be more pronounced in women with a posteriorly positioned uterus, but does not lead to a bad pregnancy.
