New surgical treatment for pelvic organ prolapse

Pelvic organ prolapse is a functional defect of the pelvic floor caused by various causes of structural defects of the pelvic floor. In modern society, with the aging of the population, the incidence of pelvic organ prolapse is gradually increasing and seriously interferes with the quality of life of middle-aged and elderly women. Uterine prolapse is a common disease in middle-aged and older women, but it also occurs in younger women, especially in postpartum women, and more and more patients with uterine prolapse, especially younger women, want to preserve the uterus to maximize sexual function while avoiding major surgical trauma. The repair of pelvic organ prolapse has accounted for 40% of major general gynecological surgery in developed countries. With the in-depth study of pelvic floor anatomy, the improvement of surgical instruments and the invention of various repair materials, pelvic floor repair surgery has developed rapidly. Classical uterine prolapse surgeries, such as cathartic hysterectomy, have been gradually replaced by newer procedures due to high trauma and recurrence rates. Modern principles of pelvic floor reconstruction surgery include restoration of anatomical structure and function, repair of defective pelvic floor tissues, rational application of alternative materials and embodiment of minimally invasive. The appropriate surgical procedure should be selected based on a comprehensive analysis of the patient’s age, physical condition, comorbidities, sexual life requirements and previous treatment. In recent years, the role of laparoscopy in the treatment of pelvic floor disorders has received increasing attention and research due to the development of minimally invasive surgery. In addition to the advantages of minimally invasive, fast recovery and light pain, laparoscopic surgery also has a good view of the pelvic anatomy and makes it easier to identify pelvic floor defects, which is a good reflection of the fact that “the appropriate method is the best method” in the selection of surgical methods. Laparoscopic uterine suspension with anterior sacral fixation for pelvic organ prolapse can preserve the uterus and restore the normal anatomical position of the uterus and vagina to the greatest extent, which is simple, minimally invasive and has good clinical results.
