Cervical ankylosis refers to spasmodic contraction and pain in the neck muscles caused by stimulation of the nerves innervating the neck muscles, neck stiffness, restricted movement, impedance sensation when passively flexing the neck, and inability to keep the jaw close to the chest. Clinical diagnosis: 1. Most of the symptoms are unilateral, slightly more in men than in women. 2. The main symptom is neck pain and restricted movement, ranging from pins and needles pain in mild cases to knife-like or tearing pain in severe cases. The pain is mainly in the neck, but can also vaguely radiate to the head, back and upper limbs. The pain can be aggravated by any activity, so that both shoulders turn when the head is turned. 3. Symptoms are low back pain, stiffness in the morning, and intermittent pain in the lower back, which are the most common manifestations of cervical ankylosis. This is the most common manifestation of cervical ankylosis. In the early stage, the manifestation is much milder and may only be low back pain. Lower back pain occurs slowly, with a dull pain that is indistinct and sometimes involves the buttocks. The pain can also be severe, concentrated around the sacroiliac joints, either bilateral or unilateral at first, but becomes bilateral after a few months. The diagnosis can be established based on the history of sudden onset, history of mild trauma and local physical signs. 4. There is no skin injury, and on examination, there is obvious pressure pain at the damaged muscles such as the trapezius muscle, which is widespread, and sometimes the pressure pain can be multiple, with mild local swelling, and the patient’s head is often tilted to one side, so it is also called “traumatic squint”. There is no positive finding in the neurological examination. Common treatment methods are: 1. General treatment 1. In the acute stage, bed rest and a diet rich in calories and vitamins should be given. Or give ATP, coenzyme A, adenosine, cytidylcholine and other drugs to promote the recovery of neurological function. A small amount of fresh plasma infusion from healthy people can also help improve the patient’s immune function, which is beneficial to the prevention of infection and recovery. 2. Turn the patient regularly, keep the skin clean and dry, and pay attention to massage the pressurized parts to prevent the occurrence of decubitus ulcers. 3.If urinary retention is severe, catheterization is required. A sterile catheter can be left in place and urine is released once every 3 to 4 hours to prevent bladder contracture. Pay attention to the prevention of urinary tract infection during the period of indwelling catheterization. For those who have difficulty in defecation, clean enema or laxative should be used in time. Adrenocorticotropic hormone Currently, it is believed that cervical ankylosis is related to autoimmunity, so adrenocorticotropic hormone treatment can be tried. Hydrocortisone 5-10mg/kg per day, added to 5%-10% glucose soluble tuck intravenously, once a day. 1~2 weeks later, reduce the amount as appropriate, or change to prednisone oral, and gradually reduce the stop. Other therapies 1. Plasma exchange: It can remove the harmful substances such as auto-circulating antibodies and immune complexes in the patient’s plasma, which can relieve the symptoms in critically ill patients, and may also be effective for those who are not effective in hormone therapy. Generally once a day, 7 days as a course of treatment. 2.UV irradiation oxygenated autologous blood transfusion: take 150-200m1 of the patient’s own whole blood and transfuse it back after oxygenated UV irradiation. 1~2 times a week for 3~5 weeks. It can promote the recovery of spinal cord function. Fourth, the recovery period treatment 1, as soon as possible to start functional exercise, pay attention to keep the limb in a functional position, to prevent the affected limb contracture or deformity. Patients with contracture or deformity should be given physiotherapy and physical therapy to further strengthen training, or small doses of Valium or Antan can be given orally to relieve muscle tone. For cervical ankylosis, timely and effective treatment is a crucial part. Of course, in addition to treatment, we should also do a good job of care measures to make the treatment more effective, and then recovery.
