Is the presence of protein in the urine a foregone conclusion?

  In the pediatric nephrology clinic, I often see many parents who are very heavy-hearted, and when I ask them, it turns out that their child’s urine protein has appeared again. “, “What should I do? When will it end?”. . Is the appearance of protein in the urine really a foregone conclusion?  Here we first recognize how kidney disease is healed. The healing of kidney disease is actually healed by the child himself, and it has little to do with the doctor’s treatment. Why do you say so? This is because Chinese medicine believes that the yang energy of the human body has the function of separating the clear from the turbid and lifting the protein, which can prevent the protein in the blood from leaking into the urine, children with nephrotic syndrome have a congenital deficiency of yang energy, and due to external factors such as colds and exhaustion, the yang energy is even more insufficient, so insufficient that it cannot perform the basic function of separating the clear from the turbid, which leads to a large amount of protein urine, and in severe cases, edema, and the onset of kidney disease. . Fortunately, most children are born with a deficiency of yang qi, but as they grow older, yang qi will continue to develop, and finally one day when yang qi reaches a normal level, they will be cured, and at this time, no matter what they have a cold or fever, no matter what they eat, no matter how tired they are, they will not have proteinuria. However, it is impossible to predict the day when the child’s Yang Qi will develop to normal. It may be related to the child’s congenital quality, acquired diseases and medication, so even if two children suffer from kidney disease on the same day, they will reach healing at different times, mainly due to the different speed of their Yang Qi development. The clinically observed time for a child’s yang energy to reach normal varies very much; short ones may be a few months, most are 1-5 years, but some children may be more than 10 years.  Before a child’s yang qi can reach normal, there must be an external yang qi to support it, so that the child’s own insufficient yang qi plus the external yang qi, i.e. the total yang qi reaches normal human level or slightly higher, so that the child does not have protein in the urine and does not develop edema. This external yang energy is hormones and herbs to nourish the spleen and kidneys.  So it is understandable why the doctor has to add hormones as soon as protein appears in the urine, because the doctor thinks that maybe the total yang energy is not enough to control the leakage of urine eggs. It is also understandable why the urine protein always appears after the hormone is reduced to a certain level. It is because when the hormone is gradually reduced and the child’s own Yang Qi is not yet up to standard, when the total Yang Qi is gradually reduced to a level below normal, the urine protein will appear, unfortunately the doctor does not know the degree of development of the child’s own Yang Qi and can only figure it out by reducing the dose again and again. When the hormone is reduced to a certain dosage, it can keep the urine protein negative, and when it is reduced further, the urine protein appears, and the doctor can determine the level of the child’s own yang energy, and at the same time determine the dosage of the child’s dependence on external hormone, and then the doctor will use this dosage for a long time, which is clinically called hormone trailing therapy, usually at least 3 months, sometimes even 1-2 years, until it is thought that the child’s own yang energy may have grown again, then another attempt is made to reduce the amount of hormones. In fact, the risk of proteinuria recurrence exists at this time, because if the doctor’s judgment is wrong and the child’s own yang energy does not go up a step, the result of hormone reduction is inevitably the reappearance of urinary protein.  A recurrence of urinary protein during hormone reduction is actually not terrible, because with a slight increase in hormone or just a return to the previous dose, urinary protein will turn negative, and it is rarely necessary to return to a full dose of hormone, and the urinary protein turns negative very quickly. The reason why this happens is that the child’s own yang energy has developed and is already much higher than when the disease first started, so naturally it does not need too much external yang energy (hormones) to support it. Therefore, the further the disease progresses, the more the child’s yang energy develops, the more stable the child’s condition will be, and the hormone support needed will become less and less, and when hormone support is not needed, the hormone can be stopped and the child will be cured by his own yang energy. So it is not that every relapse is starting from scratch and is a foregone conclusion. However, the long-term application of supraphysiological amount of hormones may interfere with the development of one’s own yang energy. For example, after using hormones, the body’s ability to produce hormones (cortisol) by itself will be reduced, while the herbs for strengthening the spleen and tonifying the kidneys will not inhibit the production of one’s own hormones, but rather promote it. Therefore, ideally, when the child’s yang is not yet developed to normal, it is best to use herbs and hormones together to support total yang, preferably with Chinese medicine treatment, and add hormones as little as possible, or without moving.  It should be noted that when the child’s Yang Qi has not yet reached full normal, even if it is already at 99% of normal, there is still a possibility of relapse, and the urine protein can reach 4+ and even be highly swollen. But this relapse may be the last one, because soon his yang may reach normal levels. Unfortunately the doctor never knows when this last 1% of yang will reach normal, maybe 1 month, maybe a few months, a year, or years are possible. This is the reason why doctors do not know how long small doses of hormones are going to drag on, and likewise TCM doctors do not know exactly how long it will take to supplement the spleen and kidneys.  In the case of hormones or/and Chinese medicine, the child’s urine protein continues to be negative, it does not mean that the kidney disease is cured, it just means that the kidney disease is controlled with medication, note that it is “controlled”, the essence of control is that the child’s own yang energy plus hormones or Chinese medicine, so that the total yang energy reaches the normal level and the urine protein does not leak out. If the hormones are stopped or if the child is hit by a cold, the urine protein may come back.  The presence of protein in the urine just means that the child is not yet cured and still needs to continue treatment, it does not mean that the condition has worsened. Before the child’s Yang Qi develops to be completely normal, it should be said that recurrence of urine protein is a norm and there is no need to worry about it at all. The number of recurrence and the severity of recurrence have little effect on the final healing of kidney disease. Parents should have a correct understanding and mindset, don’t over-treat and don’t go looking for the so-called high achievers, because the development of the child’s yang energy is unstoppable and does not depend on human will, all the doctor has to do is to give external yang energy support before the child’s yang energy develops to normal, which is to give Chinese medicine and a gradually decreasing amount of hormones to control proteinuria, which is called treatment.
