Infertility due to vulvar and vaginal disorders accounts for 1-5% of infertility. Certain organic or functional diseases of the vulva or vagina may affect the entry and storage of semen or sperm in the vagina, or may affect the normal sperm function due to changes in the environment, resulting in infertility. Transverse vaginal septum may be caused by a lack of penetration of the caudal end of the paramedian tubules on both sides where they meet the urogenital sinuses. The vaginal septum is usually located at the junction of the upper middle third of the vagina, but may occur elsewhere. Complete transverse septum is rare and may result in vaginal atresia; it usually has a small hole in the center or lateral aspect of the septum, which varies in size and interferes with the discharge of vaginal fluid and menstrual blood. Clinical manifestations: Infertile patients may have poor discharge of menstrual blood leading to dribbling, dysmenorrhea, retention of menstrual blood which may be combined with infection, painful intercourse, unsatisfactory sexual life. Diagnosis: Diagnosis can be confirmed according to clinical manifestations. Treatment: 1. Circumferential incomplete vaginal septum: in principle, radial incision and plastic surgery should be performed, firstly from the lateral wall, and a catheter should be inserted into the urethra for guidance when incising the anterior wall. The rest is the same as hymenal atresia treatment. 2, hole-like or complete transverse septum: the position and distance between the transverse septum and the cervix should be clearly explored, the transverse septum should be carefully cut under guidance and shaped as appropriate, trimmed to the septum near the base, and interrupted with absorbent sutures to stop bleeding. If the septum is high and the membrane is thick and difficult to close, a vaginal plug should be placed to dilate the area on the basis of hemostasis, covered with antibiotic oil gauze and filled. Prevention and control of infection during the perioperative period. If there are no other infertility factors, pregnancy can be normalized after treatment.
