Cervical spondylosis is caused by degeneration of the intervertebral discs of the cervical spine secondary to degenerative changes in the cervical joints, involving the surrounding spinal cord, nerves, blood vessels and other important tissues, resulting in the corresponding clinical manifestations. Vertebral artery cervical spondylosis is a common clinical type of cervical spondylosis, and there are many clinical treatment methods, but the efficacy is not exact. 46 cases of vertebral artery cervical spondylosis were treated with drug injection combined with acupuncture from June 2008 to June 2010, and good results were achieved. We report the following: 1. Clinical data and methods 1.1 General data 46 cases in this group, 26 men and 20 women; age: 38-60 years old, average 46.5 years old; disease duration 0.5-6 years. All 46 cases in this group were vertebral artery type cervical spondylosis with a history of other conservative methods of treatment for 3 months to 2 years before coming to the clinic. The group was treated with drug injection combined with acupuncture, and no drugs and other conservative treatment methods were used during the treatment period. 1.2 Diagnostic criteria according to the diagnostic criteria of vertebral artery type cervical spondylosis in the Diagnostic Efficacy Criteria for Chinese Medicine Diseases promulgated by the State Administration of Traditional Chinese Medicine: headache, vertigo, tinnitus, deafness, blurred vision, and worsening of symptoms when the cervical spine is laterally bent and backward extended. Positive rotational neck test, pressure and pain in the posterior occipital region, and reduced cervical mobility. The cervical spine open position x-ray showed that the cricothyroid gap or the crico-axial joint gap on both sides of the dentate process was unequal in width. And combined with x-ray film, CTA, DSA, CT, MRI and other examinations. 1.3 Operation method Position: the patient was placed in prone position with a low pillow in front of the neck, so that the neck was in a flat and straight state. Select the treatment point: the treatment points are mainly at the attachment points of the muscles and ligaments in the back of the neck, which are commonly 1.5-2.0 cm away from the spinous process, the upper angle and inner edge of the scapula, the posterior midline of the neck and the pressure pain points or hard nodes in the neck and neck. Generally, 8 to 10 treatment points are selected according to the patient’s neck symptoms and signs. Injection: After identifying the treatment points, mark them with gentian violet, then disinfect the neck with iodophor or alcohol and spread the towel. Connect the syringe with a No. 7 needle and inject the drug solution into the skin of the neck vertically at the treatment point, while injecting the needle, with the depth changing according to the patient’s body size, generally not exceeding 4 cm to avoid damaging the spinal cord and vertebral artery. Liquid formula: 2% lidocaine injection 5ml, trimethoprim injection 20mg, VitB12 injection l mg, VitB 1 injection 100mg, each treatment point liquid injection volume is about 1-2ml. Needle knife operation: after the liquid injection and then in the injection point for needle knife relaxation treatment. Take Hanzhang No. 4 needle knife according to the four-step needle knife operation steps standardized needle, straight to the pain point and muscle ligament attachment point, longitudinal sparing, transverse stripping, cutting and then out of the needle knife, compression to stop bleeding, band-aid to cover the needle hole, told the patient to keep the needle hole dry and hygienic for 3 days after surgery, pay attention to rest. One week later, treatment again. 1.4 Efficacy assessment criteria cure: the original symptoms disappear, the signs turn negative, can resume normal life and work, no recurrence after 3 months of follow-up; effective: the original symptoms and signs are reduced, the frequency of seizures is reduced, the duration of seizures is shorter, can participate in general labor and workers; ineffective: the symptoms and signs do not improve or worsen. The results 2.Results All 46 cases in this group underwent two courses of treatment and were followed up for 3 months to 1 year. The efficacy was evaluated according to the above-mentioned evaluation criteria, of which 30 cases were cured, 8 cases were significantly effective, 5 cases were effective and 3 cases were invalid, with an overall effective rate of 93.47%. 3.Discussion Cervical spondylosis is a common disease of the elderly. As we age, degeneration of the cervical spine, intervertebral discs and small joints leads to disc bulging or protrusion, joint edge hyperplasia and intervertebral joint instability, thus producing compression or stimulation of nerves and blood vessels. Intervertebral joint instability triggers acute or chronic injury when overloaded, producing aseptic inflammation, and inflammatory mediators are also an irritant, as well as aggravating the already existing “compression”, which in turn causes a series of clinical symptoms. The main cause of vertebral artery cervical spondylosis is stimulation or compression of the vertebral artery due to misalignment of the annulus arteriosus, resulting in spasm and distortion of the vertebral artery, causing a transient lack of blood supply to the brain and leading to symptoms such as dizziness and headache. Drug injection can promote the early absorption of inflammatory products, promote the inflammation to subside, release the muscle spasm, which is conducive to the relaxation of the patient’s neck muscles, especially the small muscle group behind the occiput; through the needle knife to release the tense muscles, ligaments and adhesions, can make the tissue structure of the cervical spine to restore the normal anatomical relationship and physiological function, and play a role in balancing the soft tissues, which can release the spasm of the vertebral artery and relieve the tension of the neck muscles, and Improving the blood flow rate of the vertebral artery, effectively relieving various symptoms such as dizziness and headache. The combination of drug injection and acupuncture release in the treatment of vertebral artery cervical spondylosis is a combination of drug and physical effects, which can play a synergistic role and make the curative effect more significant. Three cases were ineffective, including one case with a long duration of disease and heavy cervical joint degeneration, one case probably caused by overexertion at work during treatment, and one case with III degree hypertension. After the analysis of this group, this treatment method is an effective treatment method for vertebral artery type cervical spondylosis. The method has the advantages of easy operation, little pain for patients, short treatment course, easy acceptance, fast improvement of symptoms and positive effect, which is worthy of clinical promotion and application.
