Love for medicine, diligent and tireless
 A philosopher once said that life is made up of choices, and the major choices are connected into a line, which constitutes the trajectory of one’s life. Who knew that, as a famous dermatologist and scholar, Jiang Zhongmin’s career was tied to disease? And who knew that the choice he made inadvertently changed his life? From a patient haunted by disease to a doctor who saves people from suffering, Jiang Zhongmin has completed the transformation of his life with his determination and diligence.
The reason why Jiang Zhongmin embarked on the path of medicine has a lot to do with his early unfortunate experience. His father died of lung cancer when he was a child, plunging his family into financial difficulties, and at the age of 15, he suffered from severe hepatitis A jaundice, which was followed by an undiagnosed tuberculosis infection for 17 years after he was cured. The pain of his father’s death and the long-term torture of his illness made a young boy become introverted and pessimistic, but he was not defeated by reality and faced the disease head-on, developing his resilience and indomitable determination.
These unfortunate encounters sowed the seeds of medical love in Jiang Zhongmin’s heart. He longed for the day when he could become a doctor who could not only cure himself, but also help many patients who were suffering from illnesses …… Einstein once said, “The human will, supported by indomitable faith, is more powerful than those material forces that seem invincible. greater power than those seemingly invincible material forces.” Guided by his beliefs in medicine, Jiang Zhongmin overcame many difficulties and finally enrolled in the medical department of Baiqiu’en Medical University, starting his medical life.
After graduation in 1991, Jiang Zhongmin was assigned to the China-Japan Friendship Hospital of Jilin University (formerly known as the Third Clinical Hospital of Baiqiu’en Medical University). Considering his health condition at that time, Jiang Zhongmin chose the dermatology department, which seemed relatively easy. In the close contact with patients, the patients’ painful expressions and complicated conditions made Jiang Zhongmin understand that dermatology was not as easy as he thought, but also required a lot of unknown dedication and sacrifice. For this reason, he went to Nanjing Institute of Dermatology, Chinese Academy of Medical Sciences and Xijing Hospital for further training.
The time of further training made Jiang Zhongmin painful and happy. The pain is that the process of further training is not easy. When he was studying at Nanjing Institute of Dermatology, only 5 out of 100 students studied dermatopathology, and these 5 people were the most hardworking group, seeing patients during the day and reading pathology slides at night, staying up until 12 o’clock almost every day; the happiness is that dermatology is a science that has a fascinating charm, and Jiang Zhongmin was able to wander in the vast medical knowledge and enjoy drawing Nutrition ……
Looking back on the way he came, Jiang Zhongmin is very grateful that all his experiences have become a valuable asset in his life. He is grateful for the illnesses he has experienced, which have made him more calm and determined, and more able to appreciate the sufferings of his patients. He is also grateful for the mentors he met along the way – Prof. Wang Jingshan, Prof. Cheng Binzhu and Prof. Jiang Rihua, the current head of the department, as well as Prof. Sun Jianfang and Prof. Zeng Xueshi of the Nanjing Institute of Dermatology, Chinese Academy of Medical Sciences, who really brought him into the field of dermatology research and allowed him to gain a lifelong benefit of basic knowledge of dermatopathology He also gained a global perspective on the diagnosis and treatment of diseases and a pragmatic style of linking theory to practice.
With a grateful heart, Jiang Zhongmin’s medical path has become more and more determined. Over the past 20 years, he has treated more than 100,000 dermatologic cases, and he has been in charge of dermatopathology consultations in the whole department and in the province, and he has treated more than 100 cases of difficult dermatologic diseases each year, and he has accumulated tens of thousands of treatment and follow-up files. Specializing in histopathological diagnosis of dermatological diseases, Jiang Zhongmin has unique insights in the diagnosis and analysis of complex and atypical dermatological diseases, early skin tumors, immune dermatological diseases, and dermatological diseases related to systemic diseases.
Although the journey has not been easy, he never regrets his choice at the beginning. “Dermatology is the most suitable discipline for my thinking characteristics, the most precious thing I get from it is not the scientific research results or awards, but the perception and experience in the process of diagnosing and treating diseases. By constantly revisiting and sublimating these insights and experiences, I have not only saved myself from physical diseases, but also gradually stepped out of the mental shadow of my childhood.” Jiang Zhongmin said.
Nirvana sublimation in theory and practice
The skin is the most exterior of the human body, and although skin diseases can be seen, treating them, however, is not simple. There are many kinds of skin diseases, up to more than 2,000, and they are often related to the internal organs. So, how can we make a quick and effective diagnosis and treatment?
According to Jiang Zhongmin, correct diagnostic analysis is a prerequisite for effective treatment of skin diseases. In clinical practice, some cases are typical or limited to lesions of the skin itself, and applying standards and criteria may solve the diagnosis and treatment problems; however, more skin lesions are atypical of dynamic processes or lesions related to internal organ system diseases. He has encountered a situation in which a female patient of about 30 years of age with recurrent alternating erythematous edema on both upper eyelids for 5 years, with fever in severe cases, had been diagnosed by other hospitals with cutaneous-type dermatomyositis and systemic dermatomyositis, which could not be cured by hormonal treatment for many years and had caused serious adverse effects such as obesity, while various tests, including pulmonary CT, had repeatedly ruled out contraindications to hormones – -tuberculosis. Although the lung CT showed complete normalization when the patient had skin disease, and there was no image abnormality in the recent reexamination 5 years later, Jiang Zhongmin boldly shifted from typical technical indicator thinking to atypical empirical thinking after combining his own experience and the large number of follow-up cases he observed, and, with the patient’s understanding and cooperation, gave The fever was first controlled and the rash was completely relieved in the following months. This process suggested that the skin disease was related to tuberculosis at the beginning of the disease 5 years ago, and the diagnosis could be retrospectively made as: tuberculosis infection-related cutaneous dermatomyositis.
This case made Jiang Zhongmin realize that a good dermatologist should not “treat the head when there is a headache and treat the foot when there is a pain”, but should have a global concept of treating human diseases, not only looking at the histopathological changes of dermatological diseases through the surface of dermatological diseases, but also linking the potential diseases of all systems and organs in the macroscopic level; at the same time, a doctor should not rely only on books to diagnose and treat diseases. At the same time, doctors should not only rely on the dogma and technical indicators in books, but also pay more attention to digging the truth in practice and accumulating clinical practice experience in order to enhance the timeliness and foresight of experience; compared with typical and urgent complex and difficult diseases, they should pay more attention to those inconspicuous and easily ignored problems. These profound experiences have been transformed into valuable lessons and incorporated into Jiang Zhongmin’s working philosophy.
In Jiang’s eyes, skin diseases are not trivial, and sometimes inconspicuous skin diseases can become life-threatening events: a seemingly ordinary melanoma mole can be transformed into a malignant melanoma by stimulation, some skin diseases can be symptoms of diabetes, thyroid disease and visceral tumors, and skin diseases such as melanoma and lupus erythematosus are conditions with high mortality… … “The treatment of difficult diseases is not an emergency symptomatic treatment in the late serious state of the disease, nor can we just follow the standards and norms for diagnosis and treatment, but I pay more attention to the early clues that can be found in the atypical state of the disease and the correlations and etiological hints between the diseases, to discover the direction of the disease in time in the process, to analyze the dynamically changing and constantly accumulating pathogenic factors and timely attempts to intervene in these factors to change the speed and direction of the process.” To this end, he has designed the Consultation Manual, Roadmap for Clinical Thinking about Skin, and Diagnostic and Analytical Strategies for Difficult Diseases. “This allows patients to have a timely and full understanding of my work thinking and treatment characteristics. Good doctor-patient communication can bring precise treatment for patients, and also bring me continuous improvement in experience and grasp of earlier budding patterns of diseases.” Jiang Zhongmin said.
