How to treat lateral femoral cutaneous nerve entrapment syndrome by traditional Chinese medicine with local closure

The lateral cutaneous nerve entrapment syndrome refers to the inflammatory edema, adhesions and contracture irritation caused by the direct involvement of the lateral cutaneous nerve in the inguinal ligament and the surrounding soft tissue injury, as well as the sensory abnormalities in one area of the lateral femur caused by the entrapment. This disease is not uncommon in clinical practice, but it is often misdiagnosed and mistreated because of the different understanding of the disease. The author treated 23 cases from 1992 to 1995, using traditional Chinese medicine with local closure treatment, and achieved more satisfactory results, which are reported and discussed below. The minimum age was 15 years old, the maximum age was 68 years old, and the average age was 37.4 years old. There were 13 cases on the left side, 9 cases on the right side, and 1 case on both sides. The duration of the disease ranged from 7 days to 1 year, of which 17 cases were more than 1 month old. There were 3 cases of hip hyperextension sprain, 10 cases of skirt injury, and 10 other cases. There were 10 cases of lumbar disc herniation, 4 cases of sciatica, 3 cases of hip osteoarthritis and 2 cases of appendicitis, which were misdiagnosed as lumbar disc herniation before admission to the hospital. 1. 2. Symptoms and signs All of the 23 cases in this group had abnormal sensation, burning pain and paralysis in the anterolateral thigh, among which 12 cases had pain spreading to the upper patella, accounting for 52%. On examination, pressure points could be found at the lower edge of the affected inguinal ligament about 9.0 mm from the anterior superior iliac spine, and painful cords could be found at the painful points in 13 cases (about 57%), and deep finger pressure caused not only localized strange pain and numbness, but also pain running to the knee. However, the lower limb movement and tendon reflexes were normal, and the pain was aggravated when walking or standing, but alleviated when sitting and lying down. 1. 3. treatment methods and results Chinese herbal medicine was used, including 30g of Radix Rehmanniae, 30g of Radix Rehmanniae, 10g of Radix Achyranthes, 12g of Papaya, 12g of Radix Aromaticus, 9g of Peach kernel, 6g of Safflower, 12g of Sumac, and 10g of Plantago lanceolata. the above herbal medicine was decocted in water and taken twice daily, and the dregs were wrapped in cloth and applied to the affected area. For occlusion, we used 2% procaine 5m l, prednisolone 3m l, V itB 120.5mg, injected once every 5-7 days, at the painful point of inguinal ligament. In this group of 23 cases, 3 cases were treated with traditional Chinese medicine alone, 7 cases were treated with local closure alone, and 13 cases were cured by traditional Chinese medicine plus local closure. All the cases were cured by Chinese herbal medicine and local closure in 13 cases. Except for one case which was cured after 6 weeks, the remaining 22 cases were cured by symptoms and signs disappearing within 1 to 3 weeks. Seven cases were followed up from 3 to 6 months, and no recurrence was observed. The lateral femoral cutaneous nerve originates from the sensory nerve roots of L 2 to 3, travels from the lateral border of the psoas major muscle, crosses the iliacus muscle to the medial aspect of the anterior superior iliac spine, passes through the bony ligament canal formed between the two layers of the anterior superior iliac spine and the lateral end of the inguinal ligament, then turns sharply from horizontal to longitudinal, enters the thigh, descends distally on the surface or deep surface of the suture muscle, and divides into two branches, anterior and posterior. The posterior branch innervates the skin of the buttock on the upper 1? 3 of the lateral thigh and distal to the greater trochanter, and the anterior branch innervates the skin of the anterolateral thigh up to the knee. 2. 2. Mechanism of injury When the hip joint is hyperextended or externally rotated, the lateral femoral cutaneous nerve is strained at the canal orifice, or the bony ligament canal at the anterior superior iliac spine is directly bruised by external violence, or indirectly compressed by the high convex abdomen of pregnant women and obese women, causing spasm, congestion and edema of the nerve, or even inflammatory reactions such as adhesions, enlargement of the muscle lumen contents, congestion and edema, exudation and contracture of the surrounding tissues, causing the lateral femoral cutaneous nerve to The inflammatory reaction such as adhesions may occur. The main clinical manifestation of this disease is pain in the groin, so it should be distinguished from high lumbar disc herniation, appendicitis, hip osteoarthritis, etc. Although these diseases also have symptoms of pain in the groin, there is no pressure pain at the anterior superior iliac spine, and the pain does not disappear or is significantly reduced by Prudential injection. In this disease, the opposite is true, and Prudhoe’s painful point closure is an important method to confirm the diagnosis of this disease. 2. 4. Treatment principle This disease is mainly caused by the compression of the lateral femoral cutaneous nerve or stimulated by inflammation. The local closure has antispasmodic and analgesic, anti-inflammatory and anti-swelling, loosening the role of adhesions. The Chinese herbs peach kernel, safflower, hyssop and sumac can activate blood circulation and resolve blood stasis; the Chinese herbs aromatic herb, psyllium and papaya can reduce swelling and softness; the Chinese herbs elongated tendon grass and transverse bone grass can relax the tendons and activate the joints, which can reduce the degree of inflammatory reaction, shorten the inflammation period and reduce the role of connective tissue formation. The combination of the two methods can eliminate the symptoms and provide satisfactory results.
