What is polyposis? How is it diagnosed and treated?

  Polyposis is a chronic skin ulcer that occurs in the area of the calf polyposis. In ancient literature, it is also known as trouser sore, skirt wind (Zengzhi Zhunzhi) and rotten leg (Zhuanshu Zhanzhi), and is commonly called old rotten foot.  This disease is mostly seen in people who have been standing and walking for a long time, and is often a late complication of tendon tumor. It mainly occurs in the lower 1/3 of the inner and outer legs, and is characterized by the difficulty of closure after a long period of time, or it is easy to recur due to injury even after closure, independent of the season.  It is equivalent to the chronic ulcers of the lower limbs in Western medicine.  Etiology and pathogenesis: Chinese medicine believes that this disease is caused by long standing or excessive weight-bearing, which leads to transverse disintegration of the calf tendons and veins, revealing bruises and stagnant veins and veins, which turn into heat over time, or the skin of the calf is broken and infected with poison and damp heat is injected into it, and the sores do not heal over time.  Western medicine believes that the structural abnormalities of the deep and superficial veins of the lower extremities and the traffic branch veins and the increase of venous pressure are the anatomical-pathological basis for the nutritional changes of the calf skin and the occurrence of ulcers, and the poor blood return of the deep veins of the lower extremities caused by long-term deep vein valve insufficiency or the sequelae of deep vein thrombosis is the main reason for the formation of ulcers. And long-term standing, high abdominal pressure and local skin injury are the predisposing factors for ulcer development.  Clinical manifestations: This disease is mostly seen in those who stand and walk for a long time, and is often one of the late complications of tendinopathy.  Initially, the calf is swollen, pigmentation, heavy feeling, local bruises, light and heavy, aggravated year by year, or a series of venous insufficiency manifestations such as superficial phlebitis, bruising dermatitis, eczema, etc., followed by cracks in the skin of the lower 1/3 of the calf (boot area) with continuous diffuse swelling and tinea-like changes on the inner or outer side, which break down on their own or after scratching and erosion, nourishing and dripping water, ulcer formation, when the ulcer expands to a certain degree When the ulcer expands to a certain extent, the edges tend to stabilize and the surrounding area becomes red and swollen, or it does not heal over time, or it recurs frequently.  In the later stage, the sore is sunken, the edges are raised, shaped like a jar mouth, the flesh of the sore surface is gray or dark, the water is obscene, the skin around the sore surface is dark red or purple-black, or the surrounding wet sores are itchy and do not heal over time. Secondary infection is ulcerated pus, or complications bleeding. In severe cases, the ulcers may expand, reaching up to the knee and down to the back of the foot, reaching deep into the periosteum. In a few cases, the ulcers may be untreated for many years, and the toxicity of the ulcers may be so deep that they may become rocky.  Laboratory tests blood routine: the test is generally normal, a few may have an increased white blood cell count; secretion culture and drug sensitivity: to provide reference for the use of antibiotics.  The physical examination of this disease is to further understand the cause of calf ulcer, and the common clinical use includes deep vein patency test, superficial vein and traffic branch valve function test, etc. Most of the clinical methods are used to check their lower limb veins such as lower limb venous angiography and ultrasonic Doppler flow detection.  Diagnosis and differentiation: Diagnosis can be made according to the following diagnostic points, mostly in standing workers and patients with varicose veins and thrombophlebitis of lower limbs.  1. It is usually found in the lower 1/3 of the lower leg and 9 cm above the ankle bone in the inner and outer polyp.  2. At the beginning, it is itchy in the inner or outer polycarbonate of the lower leg, followed by redness and swelling, and then ulceration and dripping of water, which does not heal over time. The edge of the sore is thickened, shaped like a jar mouth, the flesh of the sore surface is gray or dark, and foul and foul pus flows out, and the skin around the sore is dark red or purple-black and shiny. The skin around the sore is dark red or purple-black. It is often complicated by eczema, and rarely has cancerous changes.  This disease needs to be differentiated from the following diseases: tuberculosis polyposis: this disease often has a history of tuberculosis in other areas; the lesions start as reddish-brown papules with central necrosis, deeper ulcers, subterranean, jagged edges, septic-like pus, purple color around the sores, stubborn ulcers, long-term difficult to heal; longer duration of the disease can be seen in the old and new overlapping scar, after healing can leave depressed pigmented scar.  1.Polyposis malignant: this disease can be primary skin cancer, but also from the polyposis after a long time, malignant; ulcers like volcanoes, the edges rolled up, irregular, hard to the touch, light gray-white, the basal surface is easy to bleed.  2, radiological polyposis: this disease often has a clear history of radiation burns; medical education|education network to collect and organize the lesion is limited to the radiation site; often from a number of small ulcers fusion into a piece, the surrounding skin has pigmentation, or interspersed with small white spots, the injured skin or muscle layer is obviously stiff, sensory weakness.  Treatment: Chinese medicine treatment of this disease is mostly due to prolonged standing or weight-bearing, resulting in the loss of smooth lower limb veins, local qi and blood stagnation, and due to damp heat injection, qi and blood stagnation, rotting flesh and flesh. Injuries to the skin of the lower limbs, insect bites and eczema are often triggering factors. It is characterized by ulcers that are difficult to close after a long time, or although closed, they are prone to recurrence due to injury.  Dampness and heat with stasis: Most often seen in the early stage of the patient, the local manifestations are several or single ulcers of different sizes in the inner and outer polycarbonate of the lower limbs, with grayish color, pus immersion, foul smell, diffuse swelling and burning around, pain and itching, or even vicious cold and fever, yellowish tongue coating and moist pulse.  The disease begins because the spleen is the master of the limbs and muscles, because of poor diet, the spleen and stomach damp-heat internal growth, or trauma damp-heat evil from the outside, damp-heat evil, blocking the meridians, Qi and blood is blocked, then local {red diffuse swelling; heat is slightly itchy, heat is painful, damp is rotten, heat is rotten, damp-heat contains steam is itchy pain rotten both see. Dampness is a yin evil, entangled and glued, so the water is dripping and the sore is rotten.  Treatment: Clear heat and dampness, and detoxify the camp.  The formula is Dioscorea Z Heterotoxicum Tang plus or minus Spleen Deficiency and Dampness: the disease is prolonged, the sore surface is dark, yellow water is drenched, or there is eczema, the affected limbs are swollen, the abdomen is distended, the stool is loose, the face is atrophic, the tongue is light with white greasy coating, and the pulse is sunken and weak.  The sore surface is not moistened because the spleen is deficient in Qi and blood, and the sore surface is not moistened because of the deficiency of Qi and blood, so the sore surface is dark in color. Yellow water immersion and swelling of the affected limbs are all signs of dampness.  Treatment: Strengthening the spleen and infiltrating dampness by adding and subtracting Ginseng and Atractylodis Macrocephalae with deficiency of qi and blood: ulceration after years, pale and stagnant flesh on the sore surface, high rise of the sore edge like a jar mouth, dark skin tone around, varicose veins in the lower legs, pale purple tongue, white and greasy moss, thin and astringent pulse.  The disease is protracted and difficult to heal, with the qi and blood being depleted over time and the spleen and stomach being weak, then the qi and blood deficiency is the main manifestation. If the sores sink because of the depletion of qi and blood, lack of nourishment and lack of source of nourishment, the sores sink. If the sore is weak, the skin around the sore is purple and dark, the flesh on the sore surface is dirty and dark, and the pus is fishy and foul-smelling, accompanied by fatigue, loss of facial color, and other symptoms of spleen deficiency and weakness of qi and blood.  1.Actively treat the original disease, eliminate the cause, such as varicose veins in the lower limbs, diabetes, etc., and consider surgical implantation if the sore surface is too large and the granulation is healthy.  2.Infrared irradiation can be used to improve local circulation as an auxiliary treatment. Such as red light therapy device, TDP and other irradiation.  3.Add antibiotics according to the culture of secretions and drug sensitivity.  4, local such as scabs thick with 4% boric acid solution hot compress or 1: 5000 hot potassium permanganate solution soaked to remove scabs, and then topical antibiotic ointment, such as Bactrim; such as ulcer secretions more available gentamicin, Alec (povidone iodine), metronidazole, etc. wet compress Chinese medicine external treatment pus more available 10% Phellodendron solution, compound Phellodendron solution, etc. wet compress; ulcer pus decay more can fly cloth Qi Xing Dan to mention pus If the pus and decay are exhausted, we can apply Pixi San or Qixing Dan externally to generate muscle and grow flesh.  For other treatments, patients can be given infusions of Astragalus injection, Shengve injection (Ginseng and wheat injection), Danshenone injection, Bitter Dish injection, Lanjianhua injection, etc. to tonify the qi and invigorate the blood.
