The occurrence and treatment of neurodermatitis

Neurodermatitis, also known as chronic simple moss, is due to a variety of reasons to the increased excitability of the neuropsychiatric system, reflected in the excitability of the skin is also increased, so the skin will react strongly to even minor external stimuli, due to the skin’s own protective function, only the skin thickened to resist the strong external stimuli, the result is that the more stimulated the skin thicker, the thicker the skin more itchy, forming a Vicious cycle. The result is that the more irritated the skin is, the thicker the skin is, the more itchy the skin is, forming a vicious circle. The nervous system excitability is affected by a variety of factors: such as work tension, excessive thinking, insomnia, fatigue, mood upset and other psychological disorders. According to the ancestral medical viewpoint is the seven emotions of happiness, anger, worry, thought, sadness, fear and fright. Clinical manifestations: At the beginning, there is only itching at the irritated area. Due to scratching and friction, flat, filled papules of different sizes appear on the skin, garden-shaped or polygonal, hard and shiny, with normal skin color or light red, distributed in patches by clusters, with unclear borders. Because of paroxysmal itching and frequent scratching, the rash gradually increases, fuses into patches, and forms mossy hypertrophy. The skin lines deepen and the skin crests are elevated. The skin also sees secondary changes: scratching, blood crusts, and peeling. Therefore, neurodermatitis often occurs in areas prone to friction. For example, the neck, elbow tips, anterior shin, waistline, coccyx, and vulva. It can also spread all over the body. Self-perceived symptoms are paroxysmal itching, which is worse in the afternoon and at night, affecting mood and sleep. Diagnosis and differential diagnosis: The diagnosis can be made on the basis of the typical moss-like changes, intense itching, predilection for the site and chronic course of the disease. However, sometimes it should be differentiated from the following diseases: 1. chronic eczema: there are more vesicles, exudates, etc. with acute onset, skin hypertrophy is obvious, skin lines are not deep, papules are not as obvious as neurodermatitis; 2. lichen planus: the rash is dark red, purplish red or normal skin color flat papules. The histopathology is specific; 3. Primary cutaneous amyloidosis: It occurs on the front of the shin of the lower leg and is a hemispherical papule of green bean size, hard and dense in patches, but not fused. Histopathology has diagnostic significance. How to treat neurodermatitis? 1, first of all, the treatment of the cause of the disease, eliminate work tension, arrange for good work and rest, sufficient sleep. Correctly deal with all kinds of things that affect the mind, lift the psychological barriers. Avoid all kinds of stimulation (such as scratching, friction, hot water washing, etc.). If you can control scratching, some of them will be fine without medication. If you can’t control scratching, use topical anti-itch medicine to stop itching, or use ice packs to stop itching locally. 2, reduce neuropsychiatric excitability: available tranquilizer, sedative, antidepressant, anti-itch drugs, such as Valium, Eszopiclone, Doxepin, chlorpheniramine maleate, Antalac, etc.. Or stronger hypnotics. 3, topical antipruritic drugs, according to the type of lesions can choose the appropriate dosage form of drugs. Such as containing menthol, camphor, phenol, ice chips, surface anesthetics, corticosteroid-type drugs. For limited skin lesions, you can apply topical skin disease treatment, which can stop itching and anti-inflammation and avoid external stimulation. The medication differs from region to region and is chosen according to local conditions. 4.Use special treatment methods: such as sleep therapy, closure therapy, encapsulation therapy, freezing, superficial radiation irradiation, magnetic therapy, wax therapy, etc. 5.Chinese medicine treatment. Chinese medicine identification, acupuncture, fumigation therapy, etc. Any treatment should be adhered to, to the skin lesions completely recede before stopping the drug, stopping early is easy to relapse. After the recurrence to early medication treatment, and so the skin thickened and then treatment will take a long time.
