In recent years, as more and more attention has been paid to the treatment of vertigo, more and more patients with vertigo are being properly diagnosed and treated. One of the topics that we must talk about is otolithiasis. The official name of otoliths is “benign paroxysmal positional vertigo” (BPPV), which is based on the etiology of otoliths to make it easier for people to call it. The otolith is a small structure in our inner ear called “stone” because of its calcium carbonate component. It has a physiological effect when it is normal, such as feeling the acceleration when they ride in a car, the acceleration and deceleration of an elevator, etc. It can cause our vertigo attacks when it is dislodged from its normal place. So, what are the circumstances to suspect that you have “otoliths”? For example, when you wake up, lie down, turn over in bed, raise your head to retrieve something, or lower your head to tie your shoelaces, you may experience a sudden, short-lived sensation of rotating objects. This is when you should be highly suspicious. Furthermore, what if the vertigo does not occur in the above cases? Or what if the vertigo is particularly long? These can be seen often in clinical practice, and if they are not typical, it is more likely to be other positional vertigo diseases, which can be identified by vestibular function tests, or other related tests. Talking more about cervical spondylosis or cervical vertigo, this is the diagnosis that people often give to their vertigo, and of course there are doctors who say this diagnosis. Patients mostly have many symptoms similar to otoliths or positional vertigo, of which many patients who come to our department for vertigo clinic have been diagnosed and treated according to this diagnosis in outside hospitals. In fact, there is no universally accepted standard for cervical vertigo at home and abroad. However, as far as our current understanding of vertigo is concerned, it may be necessary to continue to explore this cervical vertigo. Finally, let’s talk about the cost of treatment of otoliths. Recently, a survey was done by a doctor in Shanghai. Many patients with otoliths saw an average of 8 departments before they were finally diagnosed and treated correctly, and the cost was around RMB 5,000, while others spent a lot of money on injections, hospitalization, and so on. In fact, this is the result of a lack of proper understanding of otolithiasis. I will also give you some more information about it. Our treatment for otoliths is otolith repositioning, which is not very expensive. Unless after the reset the patient has a feeling of dizziness, unsteadiness, or other types of vertigo lesions that are not otoliths, some medication or further examination may be done.
