This is a case of multiple occlusion and stenosis of the carotid artery due to atherosclerosis. The patient, male, 73 years old, had dizziness and limb numbness before surgery. Imaging examinations such as color ultrasound, CTA and DSA angiography suggested occlusion of the entire right common carotid artery (occlusion length 10.5 cm), as well as combined moderate stenosis of the right internal carotid artery and occlusion of the left internal carotid artery. Usually carotid occlusion is not operable. After repeated studies and discussions before surgery, Prof. Wang Tao’s medical team designed the surgical plan closely, and the surgery was completed successfully as expected. A 10.5-cm long plaque was removed during the operation. When the last plaque at the beginning of the common carotid artery (proximal to the heart) was removed, blood flow was seen to jet out! It was confirmed that the occluded vessel had been opened and the blood flow was reestablished. He started to move around 24 hours after the operation. Postoperative ultrasound and CTA imaging confirmed that the blood flow was unobstructed and completely reconstructed without any complications. Now, 3 months after the operation, the ultrasound shows that the flow of the common carotid artery is smooth, and the patient is doing well and fully resuming daily life. This medical group has successfully completed more than a dozen cases of various different carotid artery occlusions.
