What are keratomileusis?

  What are the precautions for wearing keratoplasty lenses? The following matters should be noted.
  1, the fitting and use of keratoplasty lenses is different from other hard contact lenses, even if it is a new product, please strictly follow the prescribed procedures and requirements for cleaning, rinsing, disinfection, storage of lenses, each step can not be substituted for the other;
  2, please follow the prescribed time to the hospital for regular review;
  3. Please replace the lenses regularly according to the doctor’s recommendations;
  4. Please do not pinch the lenses to avoid damaging them;
  5, must use special care solution to take care of the lenses, forbid to use any other liquid care lenses;
  6.If you need to use other eye drops during the wearing period, please get a prescription from an ophthalmologist and follow the doctor’s instructions;
  7. If you feel eye pain, tearing, burning sensation, vision loss, etc. during the wearing period, please take off the lenses immediately and go to the ophthalmologist for examination;
  8.If the lens is closely attached to the cornea and cannot be removed, please take a drop of eye lubricant first and then remove the lens after the lens can move freely on the surface of the eye. If the lens cannot be removed, it should be handled by an ophthalmologist immediately;
  9, during the period of wearing keratomileusis, if you need an optometry, or need to wear frame glasses, please take the initiative to inform the doctor of the situation of wearing keratomileusis, to avoid the wrong optometry results;
  10.Do not take a bath or swim with keratoconus;
  11. Please do not share your keratoconus lenses with others.
  What preparations should be made before wearing keratomileusis lenses? Preparation includes
  1. Please receive instructions from an ophthalmologist on the specific fitting method;
  2. Wash your hands well with mild soap and running water before each procedure;
  3. Keep your nails neatly trimmed and not too long to avoid scratching the lenses;
  4. Care for or remove the lenses on a clean, flat, soft table so that they do not fall off on the floor or get lost;
  5. Please clean, rinse and disinfect the new lenses for the first time with care solution as required;
  6. Check the lenses for scratches, cracks, breakage or deposits before each wearing;
  7. Always take off the lenses of the same eye first to avoid confusion between the left and right, it is recommended to operate in the order of right first and then left;
  8. Clean, rinse, disinfect and soak the lenses with care solution every day in strict accordance with the regulations.
  How to wear keratomileusis correctly?
  Follow the following steps in the order of right first and then left.
  1.Put your finger into the lens case soaked with the care solution, make the lens adhere to the end of your finger and remove the lens, rinse the lens with the appropriate amount of care solution;
  2. Hold the inner surface of the lens upwards on the tip of the right index finger;
  3.Fill the inner concave surface of the lens with eye lubricant to prevent air bubbles under the lens after wearing the lens;
  4.Press the edge of the upper eyelid of the wearing eye with the index finger or middle finger of the left hand to fully support the eyelid upward and press it on the arch of the eyebrow to fix it;
  5.Press the edge of the lower eyelid of the wearing eye with the middle finger of the right hand and hold the lower eyelid downward to fully open it;
  6.Place the lens quickly and gently on the central part of the cornea (black eye) of the wearing eye;
  7. Blink gently several times and wipe away the overflowing eye lubricant with a tissue;
  8.If the lenses are pushed out of the eyes or fall on the table during the wearing process, you need to rinse the lenses with care solution before wearing them;
  9.While wearing the lenses, try to wipe away the tears or eye lotion on the eyelids so that the fingers that hold the eyelids open do not slip off and hinder the operation of wearing the lenses;
  10, after wearing the lens if found under the lens bubbles, to remove the lens, with care solution to fully rinse the lens, re-wear the lens, in the lens concave drops of sufficient amount of eye lubricant, can reduce the occurrence of bubbles under the lens;
  11. If new wearers have a foreign body feeling after wearing the lenses, they can make their eyes gaze downward and put 1 to 2 drops of eye lubricant in their eyes at the same time to help reduce the symptoms.
  How to take off the keratoplasty lens correctly?
  Follow the following procedure in the order of right first and then left.
  1. Wash your hands well with running water and neutral soap;
  2. Put 1 to 2 drops of eye lubricant in the wearing eye;
  3, fully open the upper and lower eyelids of the wearer according to the method used for lens removal;
  4.Gently squeeze the tube of the wand with the thumb and index finger to expel the air from the wand, press the disk end of the wand to the central part of the lens, release the finger that squeezes the wand to create tension between the disk end of the wand and the lens, and gently remove the lens;
  5.If the resistance when taking off the lens is high, it means that the lens is too tightly attached, you can put 1~2 drops of eye lubricant in the eye again, squeeze the disk end of the suction stick to the center of the lens, and slowly and gently move the lens to the outer white eye, it will be easier to take off the lens;
  6. After removing the lens, gently squeeze the tube of the suction tube to release the lens from the disk end of the wand and carefully remove the lens according to the shape of the disk end of the wand, do not break or pinch the lens to avoid damaging the lens.
  How to properly care for and store my keratomileusis lenses? To do the following.
  1. Care
  (1) Fill the lens case with 2/3 of the volume of care solution in advance;
  (2) Place the removed lens in the palm of the left hand and put 3 to 5 drops of care solution;
  (3) Wipe the lens with the ring finger 20-30 times, avoiding pressing the lens with the index and middle fingers to avoid damaging the lens;
  (4) Gently pinch the lens with the thumb and index finger and rinse the lens with the care solution;
  (5) Put the rinsed lenses into the lens case with the care solution and soak them for more than 4 hours;
  (6) When the lenses are not used for a long time, they must be cleaned, rinsed and disinfected strictly, and stored in the care solution, which should be changed every 1 to 2 weeks. The lenses must be cleaned, rinsed and disinfected with care solution for more than 4 hours before being worn again;
  (7) Using deprotease to clean the lenses can remove protein precipitates on the lenses, ordinary deprotease care solution cannot replace deprotease, and deprotease cleaning cannot replace regular cleaning and disinfection. It is generally recommended to use deprotease once every 1 to 2 weeks to treat the lenses, please follow the instructions of deprotease carefully.
  2. Correct use of the lens case
  (1) The lens case is an important place for microbial contamination, so please rinse and air dry the case with care solution daily;
  (2) Please clean the inside and outside of the lens case with a clean brush every 1 to 2 weeks, soak it in boiling water and air-dry it;
  (3) Please replace the lens case regularly according to the time recommended by the case manufacturer or ophthalmologist.
  3. Correct use of care solution
  (1) Care solution should be used within the expiration date, please check the expiration date on the bottle before use;
  (2) Care solution should be used within the specified time after opening the bottle, if it is used beyond the expiration date, contamination may occur, please discard the bottle;
  (3) The mouth of the bottle should not be in contact with external objects such as fingers, lenses or lens cases to avoid contamination;
  (4) After opening the bottle, please place the bottle cap with the inner mouth upwards to avoid contamination;
  (5) If you find the solution cloudy during use, stop using it immediately;
  (6) Please pour out the soaked lens solution in time and do not reuse it.
  How to arrange the review of keratoconus lenses after wearing?
  1. Time of review
  Day 1, week 1, week 2, month 1, month 2, month 3, and every 3 months thereafter.
  2. Items for review
  (1) Visual acuity test, including distance vision and near vision;
  (3) Refraction test, to detect the residual myopic refraction after keratoconus wear, and analyze the amount of refractive plasticity;
  (4) Corneal curvature measurement, measuring and recording the corneal curvature after keratoconus wear, and analyzing the amount of corneal curvature shaping;
  (5) Corneal topography measurement;
  (6) Routine eye examination, check the tear film, conjunctiva, cornea, anterior chamber, etc., find abnormalities and deal with them in time;
  (7) Corneal plastic lens examination, check whether the lens has sediment, scratch or rupture;
  (8) Lens fitting evaluation to determine if lens parameters need to be revised.
  What are the common problems after keratoplasty lens fitting? How can they be resolved? Common problems include myopic undercorrection, diplopia and corneal epithelial peeling.
  1. Myopic undercorrection
  (1) Try on again and carefully determine the appropriate AC arc parameters according to the fluorescein image and correct the flat K value parameters;
  (2) If the fluorescein image shows that the lens fits well, you can wear the lens after the re-examination and correct the spherical lens according to the degree of undercorrection;
  (3) Stop wearing the lens for 2-3 weeks, wait for the corneal curvature to recover, re-examine the lens, and increase the spherical lens according to the prescription of undercorrection;
  (4) Ask the wearer to wear the lenses regularly, 8 to 10 hours a day.
  2.Retroopia
  (1) Within 1 week of wearing the lens, the plastic area of the cornea can not fully cover the pupil, resulting in glare diplopia, continue to wear will gradually improve;
  (2) Lens deviation. Try again and increase the diameter of the lenses moderately;
  (3) The lenses are too tight. Try again and relax the flat K value appropriately;
  (4) Insufficient shaping pressure in the central area of the lens. Re-try on and correct the reversal arc of the lens.
  3. Corneal epithelial exfoliation
  (1)Within 1~2 weeks of starting to wear keratomileusis, due to the magnitude of corneal shaping, occasional corneal epithelial peeling is seen. Once this occurs, the lens should be stopped immediately until 2 days after the corneal epithelial fluorescein staining has completely turned negative. The corneal epithelium should be monitored closely for 1 to 3 days after rewearing;
  (2) After wearing the lens at night, use eye drops 3 times at an interval of 3 minutes to reduce the occurrence of corneal epithelial detachment;
  (3) Excessive lens deposits can lead to corneal epithelial peeling and require thorough cleaning of the lens with deproteinizing agents;
  (4) If corneal epithelial peeling occurs repeatedly, it is recommended to re-check the fit and relax the fit if the lens is well centered.
