The methods of facial wrinkle reduction are broadly divided into traditional surgery and newly developed improved methods or new techniques. The former is such as forehead and temporal wrinkle removal, midface and neck wrinkle removal and full neck wrinkle removal; the latter is such as endoscopic wrinkle removal, small incision wrinkle removal, laser wrinkle removal, hyaluronic acid, collagen filler wrinkle removal and so on. Various methods have their advantages and disadvantages, their indications and limitations. 1.Forehead wrinkle removal surgery: suitable for people with many deep forehead wrinkles, sagging eyebrows and upper eyelids. Forehead wrinkles are power wrinkles, which are caused by the contraction of frontal muscles, frown muscles and descending brow muscles to reach facial expression. The surgical incision is usually chosen on the hairline or within the hair. The skin and muscles of the forehead are separated, the muscles are excised or cut off, and then the separated skin with the eyebrows are lifted upward and backward, respectively. After excising the excess skin, the incision is meticulously sutured. 2.Temporal wrinkle removal surgery: temporal wrinkle removal is to eliminate or reduce crow’s feet. Crow’s feet are the power wrinkles caused by the long-term contraction of the orbicularis oculi muscle, located at the outer corner of the outer eye, around the outer corner of the eye in a radial pattern. The surgical incision can also be chosen on the hairline or within the hair. The skin is separated and the orbicularis oculi muscle is treated, then the skin is lifted backwards and tightened to remove the excess, and the incision is carefully sutured. 3.Face and neck wrinkle removal surgery: suitable for people with aging wrinkles and sagging in the lower part of the face, can eliminate or reduce crow’s feet, cheek and neck wrinkles, creases and sagging, deeper nasolabial folds, etc. The surgical incision is made along the front of the ear in a more hidden fold behind the ear. The skin is separated from the musculotendinous layer, and after the separation, the musculotendinous layer is lifted and fixed upward with greater force, and then the skin is pushed backward and fixed, the so-called “two-way pushing and tightening”, which can obtain a good “rejuvenation” effect after surgery. 4.Endoscopic wrinkle removal surgery: Through a small incision in the scalp, an endoscope is introduced, and under the direct view of the endoscope, a special tool is used to do the accurate separation of the deep surface and the cutting or excision of the muscle. Then the skin is advanced upward and backward on the scalp, lifting the excess part of the forehead, which is folded in the parietal and occipital areas. The biggest advantage of endoscopic wrinkle removal is that the incision is small, bleeding is less, and the injury is light, and the operation is more accurate and delicate because of the help of endoscopic direct vision. However, the redundant part cannot be removed, and it is expected that the redundant part is folded in the parietal and occipital areas, which can be flattened in a period of time after the surgery, and it is not noticeable during this period because it is covered by hair. In addition, because the excess part cannot be removed, it is not suitable for people with high forehead (more than 7 cm). 5.Small incision wrinkle removal: It is only suitable for those who have light forehead wrinkles between the eyebrows, loose skin or simply lifting the eyebrows. 6.Hyaluronic acid: collagen or tumescent PTFE filling wrinkle removal surgery: Hyaluronic acid or collagen injection is filled in the dermis to eliminate deeper wrinkles and folds, such as nasolabial folds, crow’s feet, longitudinal lines between the eyebrows, etc. This method is easy to operate, no major pain for the patient, and immediate effect after injection, wrinkles either reduce or disappear. However, it is only suitable for simple, localized deeper wrinkles and folds of younger age, and cannot correct sagging and aging. In addition, hyaluronic acid, collagen has the possibility of absorption and cause allergic reactions. 7.Laser wrinkle removal surgery: The principle is that through laser irradiation, the excess water in the tissue is vaporized, the aging epidermal keratin layer is eliminated, and the collagen fibers of the dermis are repaired and regenerated, thus making the skin smooth and fresh, restoring the elasticity of the skin tissue and eliminating wrinkles. Laser wrinkle removal can treat “dead-end” areas where other traditional surgical methods do not work, such as around the mouth and lips, fine wrinkles on the skin of the lower eyelids and small wrinkles on the frontal and temporal areas.
