Our menstrual cycle is divided into a follicular phase in the first half of the month and a luteal phase in the second half of the month. The onset of menstruation itself takes about 5-10 days to clear. If we wait for 3-7 days after the menstrual cycle clears before performing the hysteroscopy, it is easy to schedule the surgery in the second half of the luteal phase. Our patients may ask why the luteal phase is not suitable for surgery. It is not that we cannot have surgery during the luteal phase, but if we have surgery during the luteal phase, we then have to choose estrogen to repair the lining. The endometrium grows in the first half of the menstrual period which is in the follicular phase. If you get to the luteal phase then you have estrogen. It is like a deaf person who cannot hear speech. With estrogen, the endometrial lining is not repaired after the separation of adhesions. Otherwise the adhesions will appear again after the adhesions are separated. That is why we advise patients to come for a blood draw and make an appointment for the hysteroscopy when they have their period. Then some patients ask, “I’m on a long holiday like Chinese New Year. I have to be very prepared to have my blood drawn and make an appointment for the surgery. The hysteroscopic procedure is scheduled for the luteal phase. So what are the ways to enable us to have the surgery even after the long holidays? We can take oral estrogen supplement Glaxo on the fifth day of our period (must not be the fifth day of clean period), four tablets at a time, once a day until the appointment for the hysteroscopy procedure. What is the purpose of this? This is so that we can have the procedure done when we encounter holidays or exam days or days when we are particularly busy at work. By taking estrogen, we will be able to suppress ovulation with estrogen during the month and have a follicular phase without a luteal phase for the entire month. In addition, you should not have intercourse after your menstrual period in the month when you are preparing for the surgery. This is because anesthesia will be administered during the month of adhesion separation, and anti-inflammatory drugs will be used. This may lead to the discovery of pregnancy within a few days after the hysteroscopic surgery and the fear of damage to the fetus caused by the surgery or the use of drugs.
