Why hoarseness should not be taken lightly

During the winter and spring, when the temperature is warm but still cold, there is a high incidence of throat diseases. There are many patients who come to the otolaryngology clinic with hoarseness. The hoarseness is often neglected by patients, who always think that it is caused by cold and flu and can be treated with some “anti-inflammatory” drugs; unbeknownst to them, there are many causes of hoarseness, in addition to acute and chronic inflammation causing hoarseness, there are also organic lesions, such as vocal cord polyps, vocal cord leukoplakia, vocal cord nodules, benign and malignant tumors of the larynx, often because of the neglect of patients The best period of disease treatment is often delayed due to the neglect of patients. Therefore, unexplained hoarseness for 2 weeks or more should be promptly examined at an otorhinolaryngology clinic, especially for patients with long-term smoking and alcohol consumption. Mr. Zhu had hoarseness after a cold and didn’t pay much attention to it, so he kept taking oral cold medicine to treat it. So repeatedly more than 3 months have passed, no improvement. Mr. Zhu’s hoarseness became more and more obvious, so he went to the otolaryngology clinic for an electronic laryngoscopy and found that his vocal cords were thickened and had white spots attached. The outpatient doctor recommended that Mr. Zhu undergo surgery. Recently, Mr. Zhu underwent laryngeal laser surgery. A portion of the white mass on the right vocal cord was clamped and sent for pathological examination, followed by complete removal of the white lesion under a microscope using a high-precision ultra-pulse CO2 laser. Mr. Zhu healed quickly after the surgery and regained his loud voice. The pathological diagnosis after surgery was “vocal cord leukoplakia”. Vocal cord leukoplakia is one of the diseases that cause hoarseness. What exactly is vocal cord leukoplakia? Many people do not know. Vocal cord leukoplakia, as the name suggests, is a white patch that grows on the surface of the vocal cords. The diagnosis of vocal cord leukoplakia requires pathological examination. The pathology of vocal cord leukoplakia is a lesion of lamellar keratinization of the vocal cord epithelium, which is a change in the cells that have mutated. Vocal cord leukoplakia is considered to be a precancerous lesion and is associated with smoking, improper use of voice, chronic laryngitis, irritation by harmful airborne substances and vitamin A deficiency. Some cases have the possibility of developing into cancer, and the cancer rate is 20% to 35%. Therefore, once the presence of “vocal cord leukoplakia” is detected, it should not be taken lightly and needs to be treated actively as soon as possible. Chinese herbal medicine is unique in the treatment of vocal cord leukoplakia. Chinese medicine believes that vocal cord leukoplakia is mostly caused by the imbalance of the lung and spleen, internal phlegm and turbidity, coagulation in the throat, stagnation and proliferation, and phlegm and turbidity in the larynx for a long time; therefore, it can be treated with Chinese herbs that dissolve phlegm and disperse the knots, nourish the spleen and benefit the lung. After standardized treatment, part of the vocal cord leukoplakia can be completely faded; there are also parts of the leukoplakia that are significantly reduced in scope, creating good conditions for further surgical treatment. Vocal cord leukoplakia surgery requires microscopic “vocal cord peeling”, which means that the mucosal layer of the vocal cord is completely peeled off and removed. In addition, there are cryosurgery and electrocautery. As we know, the vocal folds are an important organ for vocalization, and the anatomical structure of the vocal folds is very delicate, therefore, vocal fold surgery is required to be very delicate. The vocal fold leukoplakia lesion is confined to the epithelial layer of the vocal folds, and the structure of the vocal folds can be clearly distinguished under the microscope, combined with high-precision ultra-pulse CO2 laser for fast and effective cutting and vaporization, the lesion is accurately removed and vaporized to eliminate, and the normal structure of the vocal folds is not damaged; the patient is less traumatized, the postoperative healing is fast, and the pronunciation function is protected. Regular follow-up is required after vocal cord leukoplakia surgery. There are many causes of hoarseness, besides acute and chronic laryngitis, vocal cord nodules, vocal cord polyps, Renk edema, vocal cord leukoplakia, and benign and malignant tumors of the larynx. Benign tumors of the larynx include papilloma and hemangioma, while laryngeal cancer is the most common malignant tumor of the larynx. The degree of hoarseness caused by laryngeal cancer is related to the growth site and scope of the tumor; in the early stage of the disease, patients often have no other symptoms but hoarseness, but as the tumor grows, the hoarseness will gradually worsen, and there will be difficulty in pronunciation, and in severe cases, there may be choking and coughing, or even difficulty in breathing and suffocation. Early laryngeal cancer can be removed by high-precision ultra-pulse CO2 laser, which is less traumatic, faster healing after surgery, less impact on patient’s quality of life, and has a high cure rate. However, clinicians found that many laryngeal cancer patients did not pay attention to hoarseness in early stage and did not go to hospital until advanced complications appeared, thus missing the best treatment time in early stage. Hoarseness is a clinical symptom and there are many diseases that can cause hoarseness; therefore, hoarseness should not be taken lightly. It is important to identify the cause of hoarseness at an early stage and treat it early. In order to prevent hoarseness caused by laryngeal diseases, we need to do the following in our daily life: 1) Quit smoking and alcohol is very important; 2) Live a regular life and rest, combine work and rest; do not stay up late or overwork; 3) Pay attention to exercise to enhance physical fitness; 4) Eat less stimulating food; 5) Master the correct way of pronunciation; 6) Once hoarseness occurs and the cause is not clear for the time being, you need to go to the otolaryngology clinic regularly for Once the cause of hoarseness is not clear, you should visit the otolaryngology clinic regularly for examination and treatment.
