Abstract: Kidney is water, the main water, kidney disease is water disease. The so-called treatment of water is the treatment of kidney disease. The left kidney is the kidney, Yin, and the right kidney is the life gate, Yang, so the kidney is the house of Yin and Yang, also known as the house of water and fire. The treatment of water is really the regulation of yin and yang or water and fire, warmth is the main method of treatment of water. Warm is the root of the treatment, the emphasis on warming Yang to benefit the Qi to transform water; Tongli method includes: pungent and warm dispersal, light seepage of water, attacking and expelling water drink, through the internal organs and diarrhea of turbidity, inducing water drink. “Ternary therapy” uses the body’s own immune function to strengthen the body, encourage Yang energy, promote the drainage of water evil, to achieve the purpose of Yang energy from the flourishing, Yin energy from the elimination. Kidney is water, the five elements are water, kidney disease is water disease. Most patients with kidney disease have varying degrees of edema, so the treatment of kidney disease is called “treating water”. In order to facilitate treatment, we divide edema into Yang water, Yin water and hidden water, and propose the first “triple therapy” – physical therapy, dietary therapy, and preferential Chinese and Western medicine for treatment, which has received better results, as described below. 1, water for the kan “water is the main moisten down”, its nature is Yin, get Yang is transformed. The clinical evidence seen by the false fire on the rushing and other diseases, patients like to drink hot soup, cold things all not, from this more sufficient to symbolize the error of nourishing Yin. Besides, the two things, the force can make up the Yang in the Kan, its nature is strong enough to eliminate the tyranny of the Yin. The most common diseases of deficiency fire, such as chronic pharyngitis, laryngitis, etc., is one of the main factors that induce kidney disease, when actively treated. However, the disease is not cured by the treatment of clearing heat and detoxification, nourishing Yin and lowering fire, such as Six Spirit Pills and Laryngitis Pills. The method of supporting yang and suppressing yin, such as licorice and dry ginger soup, and sapodilla and ritual soup and other formulas are used to treat the disease, with good results every time. 2, water classification The kidneys are on the left and right, but their role is different: the left kidney is the kidney, which is yin, and the right kidney is the life gate, which is yang, so the kidney is the house of yin and yang, also known as the house of water and fire. Under pathological conditions, when the human body is subject to external evil or dysfunction of the internal organs, especially the abnormal functions of the lungs, spleen, kidneys, and Sanjiao, the lungs are unable to pronounce and surrender, the spleen is unable to transport water and dampness, the kidneys are unable to transpire and gasify, and the water channels of Sanjiao are inaccessible, resulting in the dereliction of water metabolism, which does not follow the normal path and overflows in the skin, then the development of edema; or retained in the body cavity, such as the chest, abdominal cavity and bladder, forming hidden edema (referred to as hidden water) and the onset of disease. The onset of the disease is due to hidden edema. If you feel wind, water, dampness, dampness, dampness and heat, and the evidence is seen in table, heat and actual evidence, it belongs to Yang water; and if you suffer from dietary injury, fatigue, prolonged illness and overwork, you have a deficiency of positive energy and water and dampness are stopped inside, it belongs to Yin water. Therefore, we classify edema into Yang water, Yin water and hidden water. Different treatments should be applied to different manifestations clinically, so that the water evil has no place to hide and the normal function of the kidneys is returned and the disease is cured. 3, water treatment method treatment of water, as early as in the “Suwen – soup liquid mash sweet theory” pointed out that “flat treatment in the right balance, go to Ivana Chen s … … open the ghost door, clean the internal organs”; “Jin Kui Essentials – water gas disease” said: “all have water, swelling below the waist, when the urine; waist above the swelling, when sweating It will be cured”. According to the theory of “Blood Evidence”, “Stasis of blood transforms water into edema, which is a blood disease and water”, activating blood circulation and resolving blood stasis to facilitate water is also a good treatment for edema of long-term diseases. In short, the treatment of edema is also a matter of “pass”: either to make it sweat – for swelling above the waist, so that the watery evil is dispersed from the surface; or to facilitate its urination – for swelling below the waist, so that the watery evil is discharged from the stool; or to pass its stool – for a strong body and dry stool, so that the watery evil is discharged with the stool; or to activate it. Or invigorate its blood – “blood is water when it is not in good condition”, invigorate the blood to remove blood stasis, open up the meridians, and let the water return to the normal way; or warm its yang – for those who have been sick for a long time and have a deficiency of yang energy, yang energy will be dispersed. For example, “Jin Kui Yao – Phlegm Drinking Disease” said: “For those who are sick with phlegm drinking, warm medicine should be used and it”, which pointed out the general method of treating phlegm drinking and water dampness, and pointed out the direction for the treatment of such diseases in later times. However, for the water evil in the body cavity, i.e. hidden water, most of them adopt the method of external drainage, i.e. the category of Chinese medicine guide method. In short, regardless of the treatment method, the main method is to make the water evil have a way to go. Through years of clinical practice, reference to the literature of the past generations, summing up clinical treatment experience, we took the lead in proposing the “Ternary Therapy” for the treatment of chronic kidney disease, which has achieved remarkable clinical efficacy. The so-called “ternary therapy” refers to the physical therapy of “grasping the two ends and bringing the middle” [1], the non-invasive autotherapy of “treating the kidney with urine” [1], and the method of preferential Chinese and Western medicine. This method can maximize the mobilization of the body’s own immunity and resistance, reduce the secondary damage caused by drug treatment, focus on their own conditioning, moderate to reduce the burden of kidney metabolism. It has been clinically proven by numerous patients that this method can significantly reduce proteinuria and hematuria, improve the body’s own resistance to disease, and improve kidney function. 4. Examples 4.1 Yin water with occult water Zhou, female, 72 years old. She was admitted to the hospital on June 13, 2011 due to high edema of the limbs. Previous history of chronic renal failure for more than 7 years. At the time of admission, her symptoms included fatigue, chest tightness, abdominal distension, bilateral lower limb edema, nausea, low appetite, dry stools, and urine of about 600 ml per day, with foam. The tongue is dark red, with yellowish greasy coating and sunken pulse. On examination: Bp: 160/100mmHg, anemic appearance, low breath sounds in the left lung, wet rales can be heard at the base of the lung, breath sounds in the right lung disappeared, turbid percussion, abdominal bulge, mobile turbid sounds (), highly depressed edema in both lower limbs. A chest radiograph was taken first and reported as an occupancy, so CT examination was performed to exclude the occupancy, which was a large right pleural effusion, right pulmonary atelectasis, and large ascites. Blood biochemistry: albumin (ALB) 29.6g/L, urea nitrogen (BUN) 31mmol/L, creatinine (Cr) 558umol/L, uric acid (UA) 406umol/L. Urine routine: protein (PRO) 3+, occult blood (BLD) 1+, urine sugar (GLU) 3+. Hematology: red blood cells (RBC) 2.58×1012/L, hemoglobin (HGB) 74g/L, red blood cell pressure (HCT) 25%, TCM diagnosis: edema (spleen and kidney deficiency and internal retention of dampness). Western medicine diagnosis: 1. chronic renal failure, renal failure stage, 2. chronic nephritis, 3. chronic cardiac insufficiency, cardiac function grade 3. As the patient refused hemodialysis, he was given conservative medical treatment. Drugs were routinely applied to improve renal circulation, correct anemia, correct acid and calcium, benefit qi and nourish yin, improve coronary blood supply, diuretic and decongestant. In combination with physical therapy: massage the Baihui point on the head and the Yongquan point on the foot for 20 minutes each time, once a day, 20 times for a course of treatment. The Chinese medicine formula is selected to invigorate the yang of heart and kidney, transforming qi and promoting water. Herbs: Rhubarb 15g (later in), Danshen 30g, Radix Rehmanniae 30g (first decoction), Fried Job’s Tears 50g, Leech 10g, Di Long 12g, Du Zhong 15g, Ze Lan 20g, Pierced Mountain Dragon 30g. decoction with water, one dose per day. The patient’s general condition gradually improved. However, a large amount of pleural effusion on the right side, and the right lung function was severely impaired, affecting breathing. Therefore, localized thoracentesis with closed drainage under color ultrasound was performed, while the drained pleural fluid was routinely sent for examination, and the CEA of pleural fluid was checked several times (-), and the indices suggested that the pleural fluid was leaking fluid, and the pleural fluid caused by diseases other than nephropathy was excluded. A total of about 30,000ml of pleural fluid was released from the drainage tube, and it was confirmed by imaging that the shape of the “compressed” right lung was basically restored. The kidney function was found to be relatively stable, and the patient was discharged in good condition. In general, most of the pleural fluid is bilateral, but this patient only has it on the right side, but not on the left side, which is different from ordinary patients with pleural fluid. The mechanism of its production is related to long-term sodium retention, hypoproteinemia, and accumulation of toxic metabolites, but it may be related to its long-term right-sided lying position, and water tends to go down. For large amounts of pleural fluid, only drainage can be used. However, the indwelling tube is easily blocked, resulting in drainage failure. The reason for this is that during the retention period, the mouth of the tube is clogged due to the accumulation of proteins and glucose, which forms a film. Therefore, saline flushing should be implemented regularly to keep the orifice in a clear state so that the tangible substances in the accumulated fluid cannot come near to ensure that the indwelling tube is not blocked. Method: Every three hours, 20 ml of pleural fluid is withdrawn from the drainage tube and 30 ml of saline is injected to flush the tube and clean the orifice. Practice has proved that this method can effectively prevent the drainage tube obstruction and prolong the retention time. 4.2 Yang water with hidden water Feng, male, 25 years old. He was admitted to the hospital on November 20, 2011. He had a history of primary nephrotic syndrome for more than 1 year and had a relapse due to cold. He had a history of primary nephrotic syndrome for more than 1 year and relapsed due to cold. He had swelling around the body, weight gain of more than 20 kg, fatigue, pain in the throat, lumbago, decreased urine output of about 800 ml a day with foam, poor appetite, occasional nausea, sleep, and stool regulation. Examination: Bp: 140/95 mmHg, high edema, low respiratory sounds in both lungs, heart sounds, no murmurs, abdominal bulge, mobile turbid sounds (+), percussion pain in both kidney areas (+). The tongue is red, the coating is yellow and thick, and the pulse is smooth. After admission, blood biochemistry: ALB15.8g/L, Cr108umol/L, TC17.43mmol/L, TG13.38mmol/L. Urinary routine: GLU1+, BLD2+, PRO3+. Ultrasound showed that both kidneys were enlarged, parenchymal echogenicity was enhanced, bilateral pleural effusion and ascites. Traditional Chinese medicine diagnosis: edema (heat toxin guest pharynx, damage to the kidneys), Western medicine diagnosis: 1, primary nephrotic syndrome, 2, acute upper respiratory tract infection. Oral intake of relevant Chinese and Western medicines, static drops of blood plugging and blood sparing to improve renal blood flow. At the same time, massage Baihui and Yongquan once a day for 20 minutes, 20 times as a course of treatment; instructed to drink 150ml of mid-morning urine daily to nourish Yin and diuretic (urine of patients with nephrotic syndrome contains a large amount of protein and anticoagulant substances for absorption and utilization). The patient was given the following herbal medicine to clear the heat and promote pharynx: 20g of forsythia, 12g of scutellaria, 10g of fried gardenia, 20g of shishi wei, 20g of cicada molasses, 12g of dirong, 15g of mulberry leaf, 10g of fried bitter almond, 15g of zhebe, 50g of astragalus, 30g of fried atractylodes, 20g of fangfeng, 20g of poria, 30g of psyllium (packet), and 50g of fried barley. The patient’s weight decreased every day and edema was gradually reduced. After two weeks of treatment, weight decreased by 20Kg, edema disappeared, 24h urine protein quantification: urine volume 5500ml, protein 200mg, ALB31g/L, TC10.86mmol/L, TG3.18mmol/L. Continuing to apply traditional Chinese medicine treatment, the patient was discharged with stable improvement. Press: Diuresis is the fundamental method to eliminate edema, daily urine volume should be limited, excessive diuresis is one of the causes of NS combined with acute renal failure (ARF). Generally speaking, for patients with obvious edema, the daily weight loss should be 0.5–1Kg for patients under 60 years old, and controlled at 0.25–0.5Kg for patients over 60 years old; or the daily urine volume should be controlled at about 3000ml. However, we have observed in clinical practice that the weight can be lost by 2kg a day, and no adverse reactions have been seen as long as the herbs are combined with Chinese medicine to benefit the qi and blood, and to strengthen the spleen and kidney. Chinese medicine believes that the pharynx and kidneys are related, such as the “Ling Shu – meridians” said: “the kidney foot Shao Yin veins …… its straight from the kidney up through the liver and diaphragm, into the lungs, following the throat, holding the tongue this…”.
