OBJECTIVE: To observe the clinical efficacy of sitz bath with nitrous alum lotion in the treatment of postoperative postoperative trabecular edema of hemorrhoids. METHODS: Forty patients with postoperative trabecular edema of hemorrhoids were randomly divided into two groups. There were 20 cases in the treatment group and 20 cases in the control group. The treatment group was treated with sitz bath of nitrous alum lotion, compared with the control group which was treated with sitz bath of 0.02% potassium permanganate solution in warm water. RESULTS: There were significant differences between the treatment group and the control group in terms of symptoms and signs improvement time and cure rate (p>0.05). CONCLUSION: The efficacy of sitz bath with nitro alum lotion in the treatment of postoperative trabecular edema of hemorrhoids is accurate and worthy of clinical promotion. Trabecular edema is one of the common complications of postoperative hemorrhoid surgery, which is clinically manifested as heavy incisional pain, anal drop, patient’s restlessness, and local edema and congestion of perianal trabecular tissue. In recent years, the author used self-prepared alum lotion sitz bath to treat 20 cases of this complication, the efficacy is satisfactory, is reported as follows: 1. Clinical data The observed cases are all patients after hemorrhoid surgery in our hospital, 40 cases of post-operative trabecular edema patients were randomly divided into treatment and control groups in the order of surgery time, the treatment group of 20 cases, 11 men, 9 women, age 18 ~ 60 years old, including 13 cases of mixed hemorrhoids, external hemorrhoids Among the 20 cases in the treatment group, there were 12 males and 8 females aged 17 to 63 years old, including 15 cases of mixed hemorrhoids, 3 cases of external hemorrhoids and 2 cases of internal hemorrhoids, and the general data of age, sex and condition of the two groups were statistically processed without significant differences (p>0.05) and were comparable. 2.Treatment methods 2.1 Treatment group The treatment was carried out by sitting in the bath with nitrous alum lotion. Drug composition: 30g of mannitol, 10g of alum, 1g of ice chips, the above drug is finely ground, placed in a stainless steel basin, filled with 2000ml of boiling water to dissolve the drug completely, after the temperature of the liquid is lowered and not hot, sit in the basin and take a bath for 20 minutes each time, twice a day, once in the morning after the stool and once before bedtime. 2.2 Control group Use 0.02% potassium permanganate solution to sit in the bath with warm water, the time and method of sitting in the bath is the same as the treatment group. 3. Efficacy analysis 3.1 Observation items Pain relief time: the time when the pain is obviously reduced or disappeared; the time when the edema around the trauma disappears. 3.2 Efficacy criteria Formulated with reference to the Diagnostic Efficacy Criteria for TCM Anorectal Diseases promulgated by the State Administration of Traditional Chinese Medicine [1]. Cured: both clinical symptoms and edema around the trauma disappeared; Effective: clinical symptoms improved and edema around the trauma decreased; Invalid: no improvement in clinical symptoms and no change in edema. 3.3 Statistical methods The t-test was used for measurement data, and the x2 test was used for counting data. 3.4 Treatment results 3.4.1 The comparison of the improvement time of symptoms and signs between the two groups is shown in Table 1. 3.4.2 The efficacy results of the two groups are shown in Table 2. 4. Typical case Zhang Moumou, male, 30 years old, under local anesthesia on August 10, 2007, underwent external peeling and internal ligation of mixed hemorrhoids, and the first postoperative drug change saw a surgical wound at each of the anal margins of the truncated position 3, 7 and 11, of which the wound margins at points 3 and 11 were congested and edematous, with the edematous part The patient reported heavy local pain, and was given a sitz bath with alum lotion twice a day, and the local pain was greatly reduced the next day. 5.Discussion The local tissues of hemorrhoids are damaged after surgery, and the venous and lymphatic flow is obstructed, making the tissues around the local wound congested and edematous. This formula is easy to use, easy to accept by patients, and worthy of clinical promotion and application.
