Pre-operative precautions Do not smoke two weeks before surgery, do not stay up late, avoid colds, maintain oral hygiene, if you have any special diseases or take medication, you need to inform your doctor in advance, such as stopping Aspirin (to avoid increased bleeding during surgery), female patients should avoid physiological period. Preparation for Surgery Make an appointment with your doctor 1~2 weeks in advance for the date of surgery and hospitalization. A consultation will be arranged before the surgery. The surgeon will first take photos and evaluate the size, proportion, and symmetry of each part of the face, and complete the relevant craniofacial X-ray examination to determine the location of important nerves, which will be used to determine the location and size of the surgery. The surgeon will discuss and explain the procedure with the patient, and decide on the surgical procedure together. As anesthesia is required for the surgery, chest X-ray and blood test, urine test and electrocardiogram will be done before the surgery. If transportation is not convenient, this can be done after admission to the hospital. Hospitalization Hospitalization 1-2 days before surgery, please be sure to come on time and be sure to communicate with your physician in advance of any changes or physiological periods. Anesthesia and Surgery Craniomaxillofacial plastic surgery, with the exception of chin augmentation, is usually performed under general anesthesia. The whole process is monitored by professional anesthesiologists with precision instruments, and is carried out carefully, safely and reliably. For surgery under general anesthesia, no food or water is allowed after midnight on the day before surgery. The operation time is related to the complexity of the operation, including the recovery time before and after anesthesia and the recovery time after anesthesia. For example, if you only do jaw angle or zygomatic bone surgery, the operation time is about 2 hours, and the combination of the two will take about 4 to 6 hours. Postoperative precautions 1, postoperative catheter on the body: General postoperative will be left with an intravenous tube. Patients who undergo mandibular angioplasty will have two additional drainage tubes coming out of the wound inside the mouth, which are used to export bruises from the surgical site, reduce the degree of swelling and lower the chances of infection, and are usually removed 1-2 days after the surgery. 2. Postoperative swelling: There will be a considerable degree of facial swelling after surgery. Generally speaking, the more combined procedures and the longer the duration of the surgery, the more severe the swelling will be. As for the changes in the degree of swelling, usually in the first week the swelling is more significant, the appearance is a little unsightly, not suitable for outdoor activities; the second week or so, the swelling will be significantly reduced; after the third or fourth week in the appearance of most of the swelling is no longer obvious, but will still feel puffy, taut, unnatural. This will continue to improve over time, so don’t worry too much. Usually after 1-2 months, the swelling will be completely gone. If you need to take time off from work, we recommend a one to two week break. 3. Ice and warm compresses: For 3 days after surgery, cold towels or ice water bags can be used to apply ice to the cheeks, chin and other parts of the incision to reduce swelling. After that, you can use a warm towel to promote the subsidence of swelling, and do not overheat the compresses, which is counterproductive. 4, oral hygiene care: surgical wounds are all in the mouth, the wound infection rate is originally high, so oral hygiene care is very important. The surgical wounds inside the mouth will seep blood, so after returning to the hospital room after the surgery, you should start to rinse your mouth with water or antiseptic water, in addition to normal bedtime, depending on the situation, about every two to four hours to clean, with the use of a soft-bristled toothbrush to clean, the sooner you start brushing, the easier it is to clean. The first day after surgery will be painful and uncomfortable, but you must be patient and cooperative to minimize the chance of infection. Remember to brush your teeth and rinse your mouth after eating food other than water to maintain oral hygiene. 5. Post-operative diet: After the operation, when you are more conscious, you can try to drink water. If there is no vomiting the next day, you can start to try to drink some slightly cooler fluid, such as fruit juice, milk, etc., which can reduce discomfort. As the swelling and discomfort improve, you can gradually eat soft foods that do not require chewing, such as rice, pudding and steamed eggs. During the two weeks after surgery, you must eat fluids or soft foods. 6, skin wound care: In addition to the possible abrasion of the skin at the corner of the mouth, patients who undergo osteotomy type zygomatic osteoplasty will have a suture skin incision at the corner of the W part. After surgery, you can wash your face normally and take care of the wound with antibiotic ointment, no need to cover it with gauze, and the stitches will be removed at the clinic after about one week. 7. Oral sutures and titanium fixation plate: Oral sutures can be absorbed by the human body and generally do not need to be removed. After two to three weeks, you can start to use cotton swabs to gently clean the area around the wound to help it fall off naturally. In some cases, non-absorbable sutures are needed to close the wound, and the sutures are usually removed 10-12 days after surgery. The titanium fixation plate can be removed 8 to 12 months after surgery. 8. Normal work and rest: If you can afford it, get out of bed as much as possible, and return to normal work and rest as soon as possible, which is conducive to the early recovery of your physical and mental health. The postoperative routine must be normal and have enough rest. Remember not to go out when your physical strength is almost recovered because you do not feel well, and stay at home, indulge too much in video tapes, video games, Internet and other entertainment that affects your normal work and rest, and “staying up all night” affects your resistance (immunity), which is one of the common causes of postoperative infections.
