Diagnosis of abscess at the root of the tongue

  Abscess at the root of the tongue is a typical symptom of acute lingual tonsillitis. The lymphatic tissue at the root of the patient’s tongue is granular and elevated, congested and swollen, and is connected to the lower end of the tonsils. In severe cases, there are local ulcers and purulent secretions attached to the root of the tongue.  The main causes are Streptococcus b haemolyticus, Staphylococcus, and S. pneumoniae. Adenovirus can also cause this disease. Mixed bacterial and viral infections are also not uncommon. Bacteria may be invasive or hidden in the tonsillar fossa. When the body’s resistance is suddenly lowered by cold, humidity, overwork, weakness, excessive smoking and alcohol, or stimulation by harmful gases, the bacteria multiply and strengthen. Sometimes it is a precursor to acute infectious diseases, such as measles and scarlet fever. Acute tonsillitis is often a recurrent acute attack on the basis of chronic tonsils.  The tongue is hypertrophic, with a fat and deficient tongue, or with teeth marks on the sides, and a light and tender color. The hypertrophy of the tongue is caused by a deficiency of the yang energy of the spleen, or by a combination of cold and dampness.    Swelling and pressure pain in the hyoid bone area: swelling and pressure pain in the hyoid bone area can be seen on physical examination, and sometimes a crushed bone rubbing sound can be palpated.  The initial symptoms are upper respiratory tract infection, such as dry throat, foreign body sensation, burning and sore throat. The pain gradually increases and is more obvious on one or both sides of the tongue root, and in severe cases, it may radiate to the ear and cause severe pain. The patient is acutely ill and occasionally has high fever.  The examination shows acute congestion of pharyngeal mucosa, more mucus secretion in the pharynx and oral cavity, granular elevation of lymphatic tissue at the root of the tongue, congestion, swelling, connected with the lower end of the tonsils, local ulceration and purulent secretion in severe cases, and abscess at the root of the tongue may occur.
