On the one hand it may be due to separation of the nail in the central part of the nail bulge, or it may be a central nail bulge secondary to an infection of the perineal tissue with localised redness, pain, pressure and, in severe cases, pus, or it may be a tumour that occurs in the nail and invades the nail root, causing tissue destruction resulting in pain.
