Targeted therapy is to design the corresponding therapeutic drug at the cellular molecular level, targeting a defined oncogenic site (the site can be a protein molecule or a gene fragment inside the tumor cell). Therefore, molecular targeted therapy is also called “biological missile”. In recent years, the progress of tumor targeted therapy has entered a brand new era with the development of molecular biology technology and the further understanding of the pathogenesis from cellular and molecular levels. The progress in these fields has been rapid, and good results have been achieved in clinical practice. Nowadays, targeted therapy has become one of the main treatments for tumors. Although targeted drugs have shown relatively good efficacy in tumor treatment, they can also produce some side effects, including skin rash, chapped hands and feet, nail infection, and diarrhea. In order to maintain targeted drug therapy for the clinical benefit of patients as long as possible when the situation allows, we need to manage the adverse drug reactions promptly. Today I would like to talk about how to deal with the most common adverse reactions to targeted drugs – skin rash and chapped hands and feet. Targeted drugs such as Erythropoietin and Troche are prone to cause chapped and painful hands and feet, and should not be exposed to acidic or alkaline liquids. External washing can be used as below. Use 15g of comfrey, 30g of raw earth, 20g of Xuan Shen, 10g of Bai He, 20g of lily, 10g of mulberry leaf, one daily dose of water decoction for external washing, usually the symptoms disappear after 10 days. Many targeted therapeutic drugs can cause red rash, some very serious, part of the long in the face affect the aesthetics, available Fu Ling, forsythia, honeysuckle, bitter ginseng, Xia Gu Cao Cao, Dan Pi and other drugs, water decoction, with gauze dipped in medicine, compress on the patient. It can be applied several times a day. Case sharing 1: Gefitinib caused hand and foot cracking Wang, female, 65 years old, from Beijing. The patient’s hands and feet were cracked, and the cracked areas occasionally oozed blood, and she did not dare to touch alkaline liquid. The patient was given 15g of Comfrey, 30g of Radix et Rhizoma, 20g of Radix et Rhizoma, 10g of Radix et Rhizoma, 20g of Lily, 10g of Mulberry leaf, one dose of water decoction daily, and the cracking disappeared after 30 days, and the skin was intact as before. Case Study 2: Erlotinib caused acne on the head and face Lin, female, 48 years old, from Beijing. The main reason was that more than 3 years after the surgery of left lung adenocarcinoma, nodular shadow appeared in the lung, CEA was 110ng/ml, had 4 cycles of chemotherapy with pemetrexed, but the effect was not obvious, so she changed to oral erlotinib. After 1 week of oral administration, he had multiple rashes on the face, neck and hair, higher than the skin, red and slightly dark, less than 5 mm in size, with occasional yellowish pus spots on them, clear borders and painful to touch. His daughter laughed at his second youthfulness and reappeared with acne, and the patient was reluctant to go out because of his face. This is the blood component of damp heat, given Tu Fu Ling 30g, forsythia 10g, honeysuckle 30g, bitter ginseng 10g, Xia Gu Cao 10g, Dan Pi 10g and other drugs, water decoction, with gauze dipped in medicine, applied to the patient, multiple times a day. 14 days after the rash significantly subside, most disappeared, the skin color became dark red, no new rash, after that has been oral Erlotinib, intermittent external application of the drug, after more than 1 year without significant episodes.
