Why is there still myocardial ischemia after stenting?

Myocardial ischemia even after stenting may be caused by stent dislodgement and vasospasm. 1. Stent dislodgement: after coronary stent implantation, there is a certain chance of dislodgement, because most patients’ stents are wrapped by vascular fibers, so the probability of dislodgement is very low. After dislodgement, the supporting effect disappears, the original recanalized blood vessel is closed, and the myocardium can be ischemic again. Emergency recanalization treatment is needed. 2. Vascular spasm: due to the increase of myocardial oxygen consumption, resulting in insufficient oxygen supply to myocardial cells, the production of metabolic substances, at the same time, ischemia activates the sympathetic nerve triggered by the heart coronary artery temporary spasm, further aggravating ischemia. Daily attention should be paid to avoid factors that aggravate the increase of myocardial oxygen consumption, such as strenuous exercise and emotional excitement. When myocardial ischemia occurs, it is usually accompanied by pain in the precordial area, dyspnea, breathlessness, shortness of breath with profuse sweating, patients need to always have nitroglycerin on hand and take it under the tongue during an attack to alleviate the symptoms, and then consult a doctor in a timely manner. Specific diagnosis and treatment should follow the doctor’s advice.
