The Importance of Compression Therapy in Scar Treatment

       After plastic surgery, plastic surgeons will generally explain to patients or their families that they must apply elastic bandages to the surgical incision or implant area for pressure bandaging. It is an important part of scar treatment.  Some scholars believe that compression can reduce capillaries, parallelize collagen fibers, and reduce fibroblasts and myofibroblasts in scar tissue. Other scholars believe that compression can cause relative ischemia of local tissues, the partial pressure of oxygen in tissues decreases, the partial pressure of carbon dioxide increases, and the spiral collagen structure rearranges, thus softening and flattening the scar.  Second, compression therapy is suitable for which patients 1, large area, not suitable for radiotherapy or local drug injection patients, such as large burns patients within six months to one year after the healing of the trauma.  2, the incision scar after plastic surgery, within six months after the removal of stitches.  3, although compression therapy is not effective for active keloids, if combined with the application of compression therapy after keloid excision can reduce the recurrence rate of keloids and reduce the dose of radiotherapy and drug injection therapy.  The most effective phase of compression therapy is within six months to one year after wound healing.  2, the force of compression should be appropriate not only do not affect the blood flow of the limb, the patient can tolerate as the limit, usually the pressure can be controlled at 25 ~ 30mmHg, if the pressure is too low, it will not play a therapeutic role, if the pressure is too high, it may cause venous reflux disorders, limb edema, and even more serious complications.  3, the treatment should have a certain length of time Because of the use of compression therapy, in addition to the subjective feeling of discomfort, there is the problem of affecting the aesthetics, so a considerable number of patients do not adhere to the use of compression therapy, thus affecting the effectiveness of treatment. And prematurely stopping the use of compression therapy may cause rebound scar hyperplasia; therefore, it should be adhered to as much as possible, and if you feel the pressure is too much to bear, you can relax a little, but the stopping time should not be too long. Generally, compression therapy should be adhered to at least 3-6 months, at which time, you can see the scar congestion reduce and the bulge become flat, and after 8-12 months, most patients can stop the compression, but some patients have to insist on using it for 2 years.  Fourth, what are the specific compression methods 1, compression therapy with elastic bandage is mostly applied to patients with large burns; application of elastic fabric or outsourced fiber fabric elastic rubber band, at present, most of the hospitals that set up burns or plastic surgery have these materials, the application of these materials to the appropriate pressure wrapped around the treatment site, the method is simple, reliable results.  2.Elastic sleeve compression method This kind of sleeve can be made into mask, undershirt, glove, pants, socks, etc. according to the part of the patient’s scar, which can play a good role in preventing excessive scar proliferation for the scar on the face, limbs, etc. For the neck, chest, abdomen and buttocks, the pressure applied by such masks is difficult to achieve due to the special shape and location, therefore, the application is less effective.  3.Silicone film patch pressure method This is probably the most widely used pressure therapy in clinical practice at present. The specific practice is to apply silicone film (such as scar enemy) at the scar first, and then apply compression bandage outside. The possible mechanism of action is the reduction of water evaporation from the skin and the increase of water content in the stratum corneum under the closed environment, or it is also related to the release of low molecular silicone oil from the silicone film.  4. Sponge compression and fixation method First cut the sponge with scissors to the same size as the scar to be compressed, fix the sponge on the surface of the scar with adhesive tape, and then compress it with an elastic bandage or elastic sleeve until the scar is decongested and the scar becomes soft and flat.  V. Precautions 1. When using compression therapy, if there is trauma or blister on the surface of the scar, you should stop using it temporarily and start using it again after the above mentioned lesions are healed.  2, It is better to use it continuously until after the scar is flattened, softened and lightened in color. In order to stabilize the therapeutic effect, it should also be continued for a period of time, such as 1-2 months.  3, When treating, it is better to lined with sponge, cotton pad, gauze, etc. to avoid abrasion of the skin.  4.The elastic dressings purchased in hospitals or stores are sometimes not suitable for every patient, and sometimes, simple processing and adjustment of these elastic dressings or bandages by the patient or the patient’s family is required.  5.If the pressure is found to become smaller during the treatment, adjustments need to be made in time.  6.When compression therapy is carried out in the joint parts of the limbs, great attention should be paid to the movement of the joints and appropriate functional exercises to eliminate the occurrence of joint stiffness and other conditions.
