If you have been treated by a foreign hospital and it is medically clear that you must do IVF, please complete the following requirements beforehand: 1. Prepare the original and photocopies of the marriage certificate, ID card, and certificate of permission to have children (certificate of re-birth) for both spouses. 2. The couple has already completed the pre-IVF medical tests in the local area and brought the reports of these tests with normal or basically normal results (i.e. both partners are HIV antibody negative, syphilis serology negative or non-infectious low titer positive as certified in writing by a dermatologist/venereologist, normal liver and kidney function of the female partner, normal electrocardiogram or only minor abnormalities that do not affect the heart function, normal coagulation function).
