At 12:00 noon on October 15, the weather was a bit chilly, but in the conference room on the second floor of the Department of Radiology, nearly 30 doctors from the Department of Orthopedics, the Department of Pathology, the Department of Radiology, the Department of Nuclear Medicine, the Department of Cancer Chemotherapy and Radiology, the Department of Oncology Radiotherapy, and the Department of Hematology were gathered together to enthusiastically discuss four cases of patients with difficult or suspected spinal tumors. The consultation was attended by many big-name experts, including Liu Zhongjun, director of the Department of Orthopedics, Liu Xiaoguang, vice president of the Department of Orthopedics, Wang Junjie, director of the Department of Oncology Radiotherapy, and Zhang Weifang, vice director of the Department of Nuclear Medicine. At the consultation site, Dr. Jiang Liang, chief physician of the Department of Orthopedics, played an elaborate PPT with the basic medical history, images and pathological data of the patient. The first patient, a 38-year-old male, asked doctors from various specialties to give their opinions because of the difficulty in diagnosis. The radiologist suspected osteoblastoma, while the pathologist did not exclude hyperplasia. The second patient had a lesion at T5 and was diagnosed with epithelioid endothelioma, but there was some uncertainty about the treatment plan before, whether to remove the vertebral body directly surgically or to perform radiation therapy, and after the experts’ analysis, they finally advocated radiation therapy first to control the lesion, and then vertebral body removal if the lesion could not be controlled. The fourth patient is a woman, has run to Shanghai and other large hospitals, have failed to confirm the diagnosis, and finally recommended with a ray of hope to our hospital, the face of its special condition, the experts expressed their own views, the results are still difficult to determine, prudent, we still hope to conduct another puncture examination. This multidisciplinary consultation is mainly for cases of spinal tumors that are difficult to diagnose or difficult to determine the treatment plan in that line, and this model has been running in our hospital for more than a year (it is 4 years until 2016). Generally, one to two consultations are organized every month, and one consultation is usually 1 to 1, 5 hours, hosted by the orthopedic spine group and the radiology interventional group on a rotating basis. Spine tumors are difficult to diagnose and tricky to treat. In the past, when a difficult case was encountered, patients often took the film and ran to multiple departments, which not only delayed the time but also worked hard for the patients; meanwhile, it was troublesome to communicate between related departments. In order to facilitate patients, the disease is diagnosed as soon as possible, and the treatment plan is formulated as soon as possible. More than a year ago, under the leadership of the Department of Orthopaedics and with the joint efforts of the leaders of several disciplines, the joint consultation of difficult cases was carried out on a trial basis, and the consultation departments were expanded from the initial three disciplines to all the diagnostic and treatment departments involved. The departments and leaders involved in the consultation include experts and professors from pathology, radiology, nuclear medicine, oncology radiotherapy, oncology chemotherapy and orthopedics, and other related disciplines. Over the past 4 years of operation of the joint consultation, a total of about 615 patients have been consulted and excellent results have been achieved. It has improved the quality of diagnosis and treatment of spinal tumor patients, reduced the errors of cooperation between clinical departments, accelerated clinical turnaround, and also improved the satisfaction rate of patients. Our goal: no best, only better! We hope to treat each patient using the least invasive and most appropriate method through a multidisciplinary and collaborative approach.
