What is chronic prostatitis?

Abstract :OBJECTIVE: Chronic prostatitis (CP) is increasing, resulting in varying degrees of tension and anxiety in many patients, which are treated with the addition of antidepressants. METHODS: One hundred and ninety patients who met the diagnostic criteria were randomly divided into two groups, namely, the treatment group and the control group. In the treatment group, 110 cases were given LuoYuTai and prostate bolus; in the control group, 80 cases were given prostate bolus. Both groups were given 4 weeks of medication for a course of treatment. After the course of treatment, the drug was stopped for one week and then rechecked. Results: Treatment group: the total effective rate was 89.1%, the control group: the total effective rate was 71.3%. Conclusion: The combined application of Luyutai and prostate suppository in the treatment of chronic prostatitis has some clinical application value. The main treatment is antibacterial and anti-inflammatory, and the drugs are mainly antibiotics, but the clinical efficacy is not satisfactory [1]. From the anatomy of the prostate, it is difficult for drugs to enter; the elevated pH of the prostate massage fluid (EPS) makes 90% of the antibiotics entering the prostate non-ionic and easy to return to the plasma, resulting in insufficient drug concentration to kill pathogenic microorganisms [2]. This leads to prolonged disease duration, increased psychological burden and decreased self-control of the patient, as well as the symptoms of anxiety, fear, pessimism and other excessive stress due to long-term pain and fear of affecting sexual function and fertility. The number of such patients has increased. The results of the recent treatment are reported as follows. Data and methods 1. Clinical data 190 patients had different degrees of discomfort in urination, discomfort or pain in perineum, lower abdomen, penis and lumbosacral region, dizziness, fatigue, depression, insomnia and other psychoneurological symptoms. Cases were enrolled in accordance with the Guidelines for Clinical Research on New Chinese Medicines established by the Ministry of Health of the People’s Republic of China. Patients were excluded if they were younger than 18 years old or older than 50 years old, allergic; benign prostate hyperplasia, prostate tumor; acute bacterial prostatitis; or had used drugs for chronic prostatitis or other related drugs in the week before random grouping. 190 patients who were seen in outpatient clinics were randomly grouped according to the order of outpatient visits. The differences in age, duration and severity of disease between the two groups were statistically insignificant (P>0.05) and were comparable. Before treatment, the condition was estimated according to the NIH chronic prostatitis symptom scale, divided into mild, moderate and severe, in which the treatment group was 28, 52 and 30 cases in order, with an average (11.8±8.0) score, and the control group was 21, 40 and 19 cases in order, with an average (11.8±1.0) score. 2, treatment methods: Treatment group: oral LuoYuTai (Germany Weimar ShuPei Dr. pharmaceutical production) 300 mg, two times a day, oral, and prostate security suppository (Lizhu Chinese medicine factory production ), 2 grams, once a day, external use. In the control group, prostate suppositories (as above) 2 grams, once a day, were used externally. The patients in both groups should stop using all other related drugs during the treatment period, avoid alcohol and spicy stimulating food, and abstain from sexual intercourse. 3. Statistical treatment. The data were analyzed by Ridit analysis and X2 test, t-test. 4.Efficacy determination. Recent clinical cure: 90% reduction in symptom score compared with that before treatment, disappearance of clinical symptoms; WBC of prostate fluid examination < 10/HP; effective: 60% to 90% reduction in symptom score compared with that before treatment, basic disappearance of clinical symptoms, WBC of prostate fluid examination < 10/HP; effective: 30% to 60% reduction in symptom score compared with that before treatment, most disappearance of clinical symptoms, WBC of prostate fluid examination < 10/HP; effective: 30% to 60% reduction in symptom score compared with that before treatment, most disappearance of clinical symptoms, WBC of prostate fluid examination < 10/HP. The prostate fluid examination WBC is 25% less than before treatment; Ineffective: the symptom level score is less than 30% less than before treatment, there are still prostatitis symptoms, the prostate fluid examination WBC is less than 25% less than before treatment. Conclusion This disease has been discussed in the past, such as the Qing Dynasty. The first of these is the "white, sticky, sperm-like flow from the stem, which is not painful or astringent, and which accounts for the traces in the lower garment". It means that this disease is in the urethra dripping white at the same time, or there is burning pain in the urethra, or not painful and not astringent, which is consistent with what is seen clinically. In recent years, there has been further research on the pathology of this disease, Xue's [3] believes that most of its pathogenesis is caused by internal depression of the seven emotions, as well as the invasion of external evil and damage to the function of the internal organs. More than 50% of patients with chronic prostatitis have symptoms related to anxiety, depression, fear, pessimism, etc. Patients with prostatitis commonly have psychological disorders that manifest as insomnia, painful ejaculation, premature ejaculation, etc [4]. This indicates that psychological factors directly affect the regression of the disease, and that psychological changes are causal in the onset and regression of chronic prostatitis. Therefore, to completely cure this disease, in addition to the use of drugs for prostatitis, it is necessary to eliminate the vicious circle caused by psychological disorders. transport. This finding is evidence of the scientific validity of transrectal drug delivery for the treatment of prostatitis. Lutea is an extract of St. John's Wort, which has an activating and relaxing effect on the central nervous system and may improve depression. The active ingredients are berberine, berberine, rhubarb, etc. From the analysis of the composition of the two drugs, it has the effect of clearing heat and relieving dampness, resolving blood stasis and dispersing pain, with sedation, to achieve both the symptoms and the root cause. In summary, Luyutai combined with prostate suppositories in the treatment of chronic prostatitis syndrome has a good efficacy in reducing and relieving pain and urinary symptoms and improving neurological and psychiatric symptoms, thereby improving the quality of life, and is well tolerated, and is a new choice for the treatment of chronic prostatitis syndrome.
