Hashimoto thyroiditis (HT), also known as chronic lymphocytic thyroiditis, is an autoimmune thyroid disease. In modern society, the incidence of autoimmune thyroid disease has increased significantly due to severe environmental pollution, accelerated pace of life and work [1], and increased iodine intake due to salt iodization [2]. The disease eventually progresses to hypothyroidism, and the persistent increase in antibodies can also lead to miscarriage, infertility, and cancer in women [3-4]. There is no treatment for the cause of the disease in Western medicine, but Professor Wu Min has used a combination of Chinese and Western medicine to treat the disease and has seen miraculous results. 1, the etiology of the disease Hashimoto thyroiditis is mainly manifested as enlargement of the thyroid gland, early 20% of patients manifest hypothyroidism, 5% manifest hyperthyroidism, with the progress of the disease will eventually most patients will be manifested as plus or minus. The clinical manifestations of this disease can be classified as “gall disease” or “edema” in Chinese medicine. The disease is an immune disease and Professor Wu Min believes that it is mainly due to deficiency of the spleen and kidneys. Nowadays, people are under great pressure at work, lead an irregular life and lack of exercise, resulting in spleen injury and qi deficiency, and abnormal transportation of the spleen and stomach, manifesting as depressed mood, swollen eyelids and limbs, weakness, and menstrual disorders in women, which is the manifestation of hypothyroidism in Western medicine. In some people, because of stressful work and obvious mood swings, the symptoms are transient liver fire, irritability, hot and sweaty, protruding eyes, trembling fingers, etc., which are known as hyperthyroidism in Western medicine. As the disease progresses to the middle stage, liver depression leads to qi stagnation, spleen injury leads to qi stagnation, qi stagnation leads to fluid stagnation, spleen deficiency leads to phlegm and dampness, phlegm and qi are intertwined, blood flow is not smooth, then qi, blood and phlegm are congested and stagnated in the front of the neck, which manifests as an increasingly enlarged thyroid gland. If the patient loses treatment, the condition becomes chronic, stasis turns into heat and depletes yin and fluids, resulting in deficiency of both qi and yin; or if the condition becomes chronic, the disease causes damage to yang energy, resulting in deficiency of both spleen and kidney yang energy. If the lump increases rapidly within a short period of time, the texture is hard and the nodule is uneven, then the lump will easily become thyroid cancer. In women of childbearing age, spleen and kidney deficiency may cause damage to the spleen and kidney, or phlegm and blood stasis may intermingle in the cells and prevent the ingestion of sperm, thus preventing pregnancy. In pregnant women, deficiency in the spleen and kidney leads to lack of source of biochemistry of qi and blood, insufficient essence and blood to moisten the twins, resulting in fetal disturbance and miscarriage, or congenital deficiency of the fetus, postpartum weakness and even congenital diseases. Therefore, Professor Wu Min believes that the main cause of this disease is deficiency of the spleen and kidney, resulting in the interconnection of phlegm and blood stasis. 2, the main method of treatment in Western medicine is symptomatic treatment, such as with hypothyroidism, available thyroid hormone therapy; with hyperthyroidism, the use of thiourea or imidazole anti-thyroid drugs; small thyroid gland without obvious symptoms of pressure, the follow-up observation is not treated; for enlarged thyroid gland or thyroid nodules with symptoms of pressure and normal thyroid function The method is still controversial. Traditional Chinese medicine treatment of gall disease mostly used iodine-rich prescriptions, such as seaweed pills, kombucha pills, and seaweed yuhuodan for gall tumors, which were associated with iodine deficiency at that time. Since the introduction of the universal salt iodization regulation in 1996, iodine deficiency disorders have been largely eliminated in China, and in some places there are still problems of iodine excess. The findings of the “Iodine Causes Thyroid Disease” research group at China Medical University show that both excess iodine and excessive iodine are unsafe for the health of the thyroid gland. Therefore, the treatment of Hashimoto’s thyroiditis with thyroid nodules should not be completely based on ancient prescriptions. The modern treatment of Hashimoto’s thyroiditis with Lei Gong Doside Tablets, Kunming Shan Hai Tang and Bee Venom has been effective, but its long-term application is limited due to the toxic side effects of these herbs. Hashimoto’s thyroiditis is an immune disease, and Chinese herbal treatment has unique advantages in regulating immune function, which can improve patients’ symptoms, lower their antibody titers, and reduce or disappear the enlarged thyroid gland and nodules. According to Prof. Wu Min, the main pathology of this disease is deficiency of spleen and kidney, and interconnection of phlegm and stasis. The treatment is based on strengthening the spleen and kidney, resolving phlegm and eliminating stasis, softening and dispersing nodules, and the herbs used can also improve the immunity of patients. The formula consists of Astragalus, Radix Angelicae Sinensis, Radix Zedoariae Sinensis, Rhizoma Curcuma, Rhizoma Atractylodis Macrocephalae, Rhizoma Polygonati, Rhizoma Ginseng and Rhizoma Polygonati. The whole formula is effective in strengthening the spleen, benefiting the kidney, resolving phlegm, eliminating blood stasis, and softening hardness and dispersing nodules. According to the evolution of the disease, patients with obvious qi stagnation can add products to soothe liver and regulate qi such as Chai Hu, Yu Jin and Xiang Shen; those with obvious phlegm and stasis can add products to strengthen the function of resolving phlegm and eliminating stasis; those with stasis and heat can add products to clear heat and cool blood and resolve stasis such as Dan Shen, Dan Pi and Fried Gardenia; those with obvious spleen and kidney yang deficiency can add products to warm and tonify kidney yang such as Du Zhong, Chuan Zhuan and Sang Fu Sheng. The patient was postpartum. Combining the patient’s symptoms and tongue, moss and pulse at several visits, the evidence was attributed to spleen and kidney deficiency. According to Chinese medicine, postpartum women consume essence and blood, and if the patient’s spleen and kidneys are damaged by improper nourishment, spleen deficiency leads to deficiency of qi and blood biochemically, and the patient’s qi and blood are even more deficient and cannot moisten the limbs, bones and brain, resulting in weakness, dizziness and memory loss. If hyperthyroidism or hypothyroidism is evident in this disease, it can be treated with western medicine; if the thyroid nodule has obvious pressure symptoms, local pain is obvious, and the nodule is progressively enlarged, the patient is recommended to have surgery. In conclusion, this disease should be carefully reviewed, and the medication should be used flexibly to identify the evidence and choose the appropriate treatment method.
