Abstract: This case is a patient with chronic cholecystitis, who is usually tired and busy at work and has frequent discomfort in the right upper abdomen, which has now seriously affected the patient’s normal life and work. The patient was admitted to the hospital for surgical treatment and underwent laparoscopic cholecystectomy under general anesthesia after excluding contraindications to surgery. This surgery is minimally invasive, less invasive and has a quick recovery. The patient was discharged on the first day after surgery and the pain in the right upper abdomen disappeared, and he resumed normal life 7 days after surgery.
Basic information】Male, 38 years old
Disease Type】Chronic cholecystitis
Hospital】Liaocheng People’s Hospital
Consultation time】November 2021
Treatment plan】Surgical treatment (cholecystectomy)
Treatment Period】3 days in hospital
Effectiveness】Discomfort in the right upper abdomen disappeared and she was discharged successfully.
I. Initial consultation
Today, a male patient came to the clinic with an anxious face. The patient reported that he was busy and tired at work and had frequent discomfort in his upper right abdomen. After a brief communication with the patient, I explained the treatment principles of chronic cholecystitis, first of all, to give anti-inflammatory and biliary treatment to control the symptoms, if the long-term existence of chronic cholecystitis seriously affects normal life and work, you can consider surgery, and the main surgical method is laparoscopic cholecystectomy. This patient thought that chronic cholecystitis had seriously affected his normal life and work, so he decided to be hospitalized for surgery.
II. Treatment history
After admission, the patient underwent routine laboratory tests, including routine blood tests, liver and kidney function, virus screening, and coagulation mechanism, etc. None of the above tests revealed any significant abnormalities, and then the patient underwent abdominal ultrasound and abdominal CT, which revealed that the gallbladder did have a thick wall, but no clear stones or polyps were found. Long-term gallbladder wall thickening may cause more serious problems, so we also recommended the patient to undergo surgical removal. After explaining the risks associated with surgery, the patient accepted and understood, and underwent laparoscopic cholecystectomy under general anesthesia the next day.
III. Treatment results
Since laparoscopic cholecystectomy is a minimally invasive surgery with shorter operation time and less trauma, the patient was cured and discharged on the first postoperative day. After the laparoscopic cholecystectomy, the patient’s symptoms of right upper abdominal discomfort disappeared, and the patient gradually resumed normal life and work with relieved anxiety. The patient was discharged earlier because most hospitals are currently performing day surgery, but the incision removal usually takes about 7 days, and the incision needs to be changed once in about 3 days. The patient followed the doctor’s instructions for a review, and it was learned during the review that the patient’s pain symptoms had disappeared and he was now back to work. The patient was very satisfied with the treatment effect.
IV. Notes
We are glad that the patient’s condition has been relieved after surgical treatment. However, it is still necessary to pay attention to the diet. The first 3 months after surgery should be based on light, easily digestible food and avoid eating greasy food. Because greasy food may affect the digestive function after cholecystectomy, and the gallbladder loses the function of storing bile at this time, patients may eat greasy food that may cause bile excretion too fast, thus causing diarrhea. However, the organism will soon adapt to this process and the digestive function will gradually be normalized. In addition, patients should pay attention to the condition of the incision. If the incision is red, swollen or infected, they should come to the hospital for review in time; if there are no other abnormalities, they can come to the hospital for review about 1 month after the operation.
V. Personal insight
With the increasing pace of life and changes in diet structure, the incidence of chronic cholecystitis is increasing and tends to be in young people, mainly related to strain, stress and unhealthy diet. The main symptom of chronic cholecystitis is pain or discomfort in the right upper abdomen, which is usually treated with anti-inflammatory and cholestatic treatment, but more importantly, lifestyle adjustment. The actual fact that many patients are unable to change their lifestyles due to work causes the symptoms of chronic cholecystitis to recur, seriously affecting normal life and work, this time you can choose surgery, the current surgery is minimally invasive treatment, laparoscopic surgery is the main, with good results, fast recovery, less impact.
