The tear trough is a distinct groove that forms in the junction area between the lower lid and facial skin. Generally speaking, when you are young, the skin is firm, the collagen and subcutaneous fat of the eyelids and facial skin are rich, and the boundary between the two is not too obvious (Figure 1), but as the difference between the two becomes greater with age, the boundary gradually becomes obvious and the tear trough appears (Figure 2); now, with the rapid development of mobile Internet and smartphone users, modern young people apply their cell phones for too long and Especially in the evening, not enough rest time, also prematurely appear tear troughs, and this trend is rapidly developing. The appearance of tear troughs can make people look tired and aging.

Also, the appearance of tear troughs tends to make bags under the eyes look more prominent. Some young people mistake the appearance of tear troughs for a request for eye bag removal simply because they appear and the lower eyelid above them looks bulging. Likewise in some people who have already had surgery, they mistakenly believe that the doctor has not removed the bags because the tear trough has not improved. The root of these problems does not lie in the bags themselves, but in improving the tear troughs, which will improve significantly once the troughs are filled in (see my series of articles on autologous fat transfer facial rejuvenation cases for more details).
As for the tear troughs in young people, the best method to choose is injection, specifically autologous fat, hyaluronic acid and collagen, among which, autologous fat is the best method

The reasons are as follows.
1. Since tear troughs are caused by atrophy and reduction of subcutaneous fat, autologous fat grafting/filling is the most appropriate from targeting the cause of the disease.
2. Autologous fat is autologous and there is no rejection, etc.!
3. Autologous fat is permanent, no need to inject repeatedly, reducing the number of pain!
4. autologous fat is self, there is no risk of infectious diseases, etc. brought by other foreign bodies!
5. More importantly, autologous fat is a living tissue with stem cells and other beneficial active factors that can improve the local skin quality.
For older people with both bags and tear troughs, the best solution to fill the tear troughs is to perform orbital fat release to fill the tear troughs at the same time as the eye bag surgery (as shown below).
In addition, based on my clinical experience for so many years, I found that for young people who have both mild bags and tear troughs, the best method is to use a unique method to release the orbital rim ligament when doing autologous fat filling for tear troughs, so that not only the tear troughs are well corrected after surgery, but also most of the bags disappear or improve significantly. This is perhaps the most perfect method for this case, and another extension and development of my application of this technique of fat grafting!
