Abstract: The child often presented with nasal congestion and runny nose, along with decreased sense of smell and tinnitus, and self-medication, but the symptoms were never relieved, so he came to the clinic and was examined and found to be a clinical manifestation of nasal polyp. Nasal polyp is a benign nasopharyngeal tumor. The cause of nasal polyp is usually not clear, but may be related to pathogenic microbial infection and genetic factors. After aggressive surgical treatment, the child’s nasal polyp was removed and the headache and runny nose disappeared.

I. Initial consultation
A 12-year-old child accompanied by his parents came to the clinic. The child was slightly irritable and kept blowing his nose. The child’s parents reported that since 3 months ago, the child often said that his nose was not clear and he always had a runny nose, but the parents thought it was a simple rhinitis and gave the child cold medicine on their own, but the symptoms never subsided. Recently, the child’s sense of smell became worse and there was a ringing sound in his ears, so he came to the hospital. After examination, the child’s nasal cavity was found to have mild enlargement of bilateral inferior turbinates, white lychee-like neoplasm in the middle nasal passage with purulent secretion around it, and no obvious pressure pain in the sinus area, which was initially diagnosed as bilateral nasal polyps.
II. Treatment history
We performed electronic nasopharyngoscopy on the child and saw translucent polyp-like material in the nasal cavity, which confirmed the diagnosis of bilateral nasal polyps, and suggested surgical excision and treatment. The child was then subjected to routine preoperative tests such as routine blood, urine, electrocardiogram, liver and kidney functions, etc. After excluding other contraindications to surgery, further surgical treatment was performed.
Bilateral nasal polypectomy was performed under general anesthesia, and the colored lyrium-like neoplasm was removed. After surgery, the child was given cefadroxil tablets for anti-infection treatment and could be discharged after good recovery of nasal ventilation function.
III. Treatment effect
The child’s symptoms of nasal congestion and runny nose were improved after active surgical treatment. On the second day after surgery, the child’s symptoms of nasal congestion and runny nose improved significantly, and the symptoms of tinnitus disappeared and the sense of smell returned to normal, and there was no infection on the surgical wound. On the 5th day of hospitalization, the child’s symptoms of nasal congestion and runny nose were completely relieved, and the prognosis of the surgical wound was also relatively good, so the treatment was relatively satisfactory. At the postoperative review, the child’s nasal symptoms did not recur and the prognosis was relatively good.
IV. Precautions
I am happy for the child that his nasal symptoms have improved and he has recovered well after the surgery. After discharge from the hospital, the family should pay attention to cleaning the nasal cavity of the child, and use physiological saline for flushing to avoid recurrence of irritation. They should also pay attention to keeping the nasal cavity warm to avoid inflammation caused by cold. In addition, the child should undergo regular review after discharge to clarify the prognosis.
V. Personal insight
The family of the child in this article should be warned that when the child has symptoms such as nasal congestion and runny nose, they did not wait for the child to go to the hospital for examination in time, but blindly used medication on their own, which led to the aggravation of the child’s condition and the need for surgical treatment. Generally, when the nasal polyp first occurs or the volume is small, the symptoms can be improved by nasal spray or oral glucocorticoids and other drugs, not necessarily require surgery, so when the symptoms of discomfort, it is recommended to go to the hospital in a timely manner, do not blindly self-medication, so as not to delay the condition of the symptoms worsen.
