Chinese medicine treatment is divided into: internal and external treatment – unique Chinese medicine formula (internal + external), internal treatment is mainly for the gradual removal of bacteria from the bone tissue from the inside out, the removal process is slow, in patients without sinus tracts, i.e. patients with sclerosing osteomyelitis, the treatment process is slower and the treatment time is in about 3-4 courses; but if the patient has sinus tracts, the treatment process is shortened and the treatment time is in 2-3 courses. During the treatment process, the patient’s sinus tract opening will gradually become smaller and the secretion will gradually decrease and become thinner as the bone bacteria gradually become less and less. During the treatment process, there may be an illusion of wound healing, and this phenomenon usually occurs around 6 months of treatment. With the prolongation of treatment, the secretions from the diseased bones have reaccumulated in the body, and after accumulating for a certain period of time, the wound will ulcerate again. Clinical manifestations of the affected area are painful and uncomfortable. Sometimes this phenomenon can occur 2-3 times. This is the reason why I do not allow the patient’s sinus opening to be closed in the first six months of treatment. The use of topical Chinese medicine is actually a topical medicine that keeps the sinus opening open during the treatment process. The characteristic of Chinese medicine is that it takes a long time to treat, but its advantage is that it treats from the inside out, the treatment is complete, and there is no bacterial resistance. After the cure, there is no recurrence. Western medicine treatment is also divided into internal and external treatment: internal treatment mainly consists of the use of various antibiotics, which are very effective in the short term, but with the use of up to 3 weeks, the bacteria gradually develop resistance, so the antibiotics have to be changed frequently. External treatment mainly includes various kinds of debridement, drug exchange, closed drainage or VSD negative pressure drainage and other treatment from the outside to the inside. The advantage of this method is mainly that it can all remove necrotic soft tissue and lesioned bone tissue in a very short time. After a large amount of lesioned bone tissue is clear, only a small amount of bacteria exists in the bone (which is very difficult to remove), and with the application of antibiotics, a small amount of bacteria is further inhibited (but not completely killed), and the wound can generally heal within 2-3 weeks without the stimulation of lesioned bone secretions. At this point, Western medicine becomes clinically cured. After healing, it is recommended to take oral antibiotics again for a certain period of time. In fact, the bacteria in the body are not completely killed, and when the patient is weak, the bacteria will return and the affected area will re-form sinus tracts. The wound is cleared again, the bone is scraped, and it heals again. The average weak osteomyelitis patient may have to undergo 2-3 surgeries in their lifetime, but eventually Western doctors will give up on the debridement treatment and recommend amputation or seek Chinese medical treatment. The main reason for this is that there is very little bone left. Of course patients with stronger constitution will generally have a longer time to relapse, with 20-30 years to relapse. There are also recurrences in 4-5 years. Chinese and Western medicine treatment features: both the combination of the above two methods, the best results. There are two specific cases: surgery is performed first and post-operative treatment with both Chinese and Western medicine is performed. That is, antibiotics are used along with internal Chinese medicine treatment. The healing time is fast, between 1-2 courses of treatment. Chinese herbal medicine treatment is performed first, and then surgery is performed when the patient is treated for about 2 courses of treatment. You can continue to take herbal medicine after surgery, which is better and has a stronger guarantee of cure. Of course, one can also stop taking herbal medicine. The cure rate is also higher, but not as good as the former. However, each method also depends on the actual situation of the patient and the financial situation. If the patient does not have large pieces of dead bone (diseased bone is not dead bone) dislodged, conservative treatment can be performed. On the contrary, surgical debridement is required.
