After the fetal head was delivered, the fetal anterior shoulder was found to be embedded above the pubic symphysis. The diagnosis of obstructed shoulder delivery was made, and the mother was immediately given a lateral perineal incision to widen the exit. She was diagnosed with obstructed shoulder labor. She reported that her last menstrual period was on June 2, 2021, and her menopause was over 1 month after her self-test for positive urine HCG. On March 9, she had no contractions or redness at 40 weeks of pregnancy, and her fetal movements were normal. She felt occasional lower abdominal distension, no redness, and still no obvious signs of labor, and was recommended to be hospitalized to induce labor. After admission to hospital, blood tests, electrocardiogram and fetal ultrasound were completed. The ultrasound indicated that the fetus was in occipital left anterior position and the weight assessment was about 3880g. As she was 41 weeks pregnant, the fetus was assessed to be overweight based on ultrasound results and abdominal measurements, but her height and birth canal were evaluated and she was given a trial of labor under close monitoring, to which she agreed. At 15:00, the vaginal examination showed that the opening of the uterus was 2 cm and the fetal head was -2. The labor progressed smoothly, and at 22:40, the vaginal examination showed that the opening of the uterus was complete and the fetal head was +1. The midwife immediately gave a lateral perineal incision and instructed the mother to bend her thighs as close to her abdomen as possible, but the fetal shoulder was still embedded in the pubic symphysis. The doctor found the fetal anterior shoulder above the pubic symphysis and applied downward pressure, and the midwife cooperated to help pull the fetal head, and soon the fetal anterior shoulder passed the pubic symphysis and the fetus was delivered successfully. The fetus was delivered successfully with the help of medical staff. The weight of the fetus was 3910g, and the fetus had an Apgar score of 8 at 1 minute. The midwife examined the maternal birth canal and found a laceration at 5 points of the cervix and a laceration at 2 points of the vagina, but they were not serious and were given careful sutures. The occurrence of obstructed shoulder delivery is unpredictable before delivery, and it can also occur in normal weight fetuses, and the chance of occurrence increases for the fetus with large weight in this article, so the progress of labor should be closely observed, especially after the delivery of the fetal head, the front shoulder of the fetus will be embedded above the pubic symphysis, which will prevent the delivery of the fetus, so we should be highly alert to obstructed shoulder delivery.
