What are the things to pay attention to after iodine 131 treatment for hyperthyroidism?

  Iodine-131 treatment is the preferred treatment for hyperthyroidism in adults. It has the advantages of simplicity, wide scope of application, safety and effectiveness, short cure time, low cost and very few recurrences, and no long-term adverse consequences except for the possibility of hypothyroidism. Iodine-131 treatment does not reduce the fertility of patients, is not carcinogenic, and has no effect on the development of offspring when used before pregnancy. The Department of Nuclear Medicine of our hospital is the only specialty in Jiangmen Wuyi area that legally carries out isotope iodine-131 treatment for hyperthyroidism. From the beginning of 1997 to November 2015, nearly 20,000 patients with hyperthyroidism have been treated, and very good treatment results have been achieved.  Precautions before and after taking iodine-131: 1. Pregnant and breastfeeding hyperthyroid patients are prohibited from taking iodine-131 treatment. Patients who currently have high fever, frequent diarrhea, vomiting and heart rate over 140 beats/min should not be treated with iodine-131 immediately. If you have any special discomfort when taking the medication prescribed by our doctor recently, please explain to the doctor in advance.  2. Fast for at least 2 hours before taking the drug and eat solid food only 2 hours after taking the drug, and drink more water on the day of taking the drug.  3.After taking Iodine-131, you can leave the nuclear medicine department. Patients who have vomiting and motion sickness on a long journey should stay nearby for one night before going home, or at least go back that afternoon.  4. Two weeks after taking the drug, you should rest more, avoid strenuous exercise, staying up late and heavy physical labor, and engage in light physical labor (such as housework, sitting in the office, etc.). If you need to write a leave slip and a certificate, please take the medicine before (in advance!) Ask your doctor to write a leave of absence certificate. If you have to travel by airplane or go through strict security check within one month, please also explain to your doctor.  5. Do not press or press the thyroid gland after taking Iodine-131, you can wipe sweat or itch.  6.After taking Iodine-131, pay special attention to avoid mental stimulation (such as quarrels) and prevent infection. If you have a cold, fever, stomachache or other discomfort, you can go to the doctor for medication, which will not affect the efficacy of the treatment. If you need to use Chinese herbal medicine, please ask your doctor to confirm whether there is iodine-containing herbal medicine.  7. Avoid eating iodine-containing food (such as seaweed, seaweed, nori, all seafood) and not rubbing iodine before hyperthyroidism is cured after taking iodine-131. Ordinary iodized salt contains only a trace amount of iodine and can be used as usual (non-iodized salt is not recommended).  8. Minimize contact with pregnant women and children under 12 years old within two weeks of taking iodine-131, and do not sleep in the same bed with them for 1-2 weeks. Avoid contact for more than half an hour and within 1 meter during normal times! Adults or adults without any requirements can live together as usual. For men and women with reproductive needs, contraception must be used for 4 months after iodine treatment, and pregnancy is not recommended until hyperthyroidism is cured.  9. A few patients may experience a transient aggravation of the disease 1-4 weeks after treatment, so please take complementary therapeutic drugs (already prescribed by your doctor) at the same time, but how to develop ① high fever (over 39℃), ② frequent diarrhea, ③ vomiting and ④ heart rate over 140 beats/min symptoms (when any one of them!) within 1 month after treatment Please go to your local hospital for treatment or contact our department as soon as possible (by WeChat or phone)! (Please bring all your medical records and inform your doctor that you have taken iodine-131 for hyperthyroidism, especially if the dose exceeds 15mCi).  10. 1-2 weeks after taking iodine-131, you may experience pins and needles in the neck, swelling and pain in the thyroid gland, nausea, weakness, dizziness, etc. These are treatment reactions, which usually disappear after 1-2 weeks and do not require special treatment.  11. After 3-4 weeks of iodine treatment, some people may experience cramps, numbness of hands and feet, etc. They may take calcium tablets or drink more milk.  12.Iodine-131 is usually effective after 2-4 weeks, and the symptoms improve significantly or disappear after 2-3 months, and more than 90% of patients will be cured in about 6 months.  13. In order to better understand your condition and observe the efficacy of treatment, please make sure to return to our department for follow-up after 1 month of taking the drug (please bring your medical records).  A. Please continue to use the medication prescribed by our doctor according to the dosage (only 2 times on the day of iodine treatment), and stop the one you have finished eating! Review after one month. Some of the drugs may be added or stopped in advance, so please take them according to the doctor’s requirements and check again after one month. If you feel uncomfortable or your condition changes after taking the medication, please feel free to check again in advance.  B. Notice for review: After iodine-131 treatment, the first review is 1 month later (you can pay attention to the review time on the back of the medical record or weibo reminder), usually 2-3 days after 1 month (it is recommended that there are fewer people on Tuesday, such as time conflicts can be a day earlier or delayed). The first recheck can be done with breakfast (usually no blood draw). Subsequent re-examinations are required for major blood tests 3 months, 6 months and 1 year after iodine treatment. It must also be noted that hyperthyroidism requires lifelong follow-up, even after complete cure, at least once every 1-2 years.  C. With the development of mobile health care, it is recommended to make an appointment (more than one day in advance) before your follow-up appointment to reduce your waiting time. If weakness in hands and feet occurs before and after treatment, if it is not due to hunger, please promptly take potassium supplementation and low-sugar diet (you can eat more high potassium foods such as plantain, orange, orange and millets, etc., do not drink bottled saccharin drinks, and eat less desserts and sweet fruits).  E. Since there is a clear correlation between hyperthyroidism and smoking, it is recommended that patients with proptosis must quit smoking.  F.
