New idea of acupuncture combined with antidepressants for premature ejaculation

  Many patients with premature ejaculation, both primary and secondary, are generally prescribed antidepressants, but there are no indications for premature ejaculation in the instructions for these drugs. Adverse effects at the time of treatment are generally rare and tend to occur in the first week of treatment and disappear after 2-3 weeks of continued treatment; these include fatigue, yawning, mild nausea, diarrhea, or sweating. Compared to depressed patients taking SSRIs, premature ejaculation patients without depression rarely report events such as decreased libido and erectile dysfunction, while rare men experience neurological adverse effects including euphoria and mild mania.  On-demand administration of chlorpromazine, paroxetine, sertraline, fluoxetine and fluoxetine 3-6 hours prior to intercourse is mildly effective and well tolerated by patients, but most studies have shown a significant reduction in delayed ejaculation time compared to daily treatment. On-demand treatment can be combined with other daily treatments or used in preliminary trials of low-dose daily treatments.  The major drawback of treatment with SSRIs or TCAs is the limitation that premature ejaculation symptoms rebound when the medication is discontinued. In addition, patients are reluctant to try medications whose instructions are not clearly labeled for the treatment of premature ejaculation. Although there is Level 1A evidence to support the efficacy and safety of daily use of the SSRIs paroxetine, sertraline, citalopram, fluoxetine, the tricyclic antidepressant clomipramine, and on-demand chlorpromazine, paroxetine, and sertraline in the treatment of primary premature ejaculation and acquired premature ejaculation. The report showed that 30% of patients refused to take them when starting treatment (paroxetine, 10 mg takes 21 days and 20 mg daily) and another 30% would ask to stop when they first started treatment. Reasons given include: not wanting to take an antidepressant; lower than expected treatment effect; temporary decrease in sexual interest due to relationship tension and side effects.  Acupuncture is a medical feature of China and plays an important role in the treatment of various diseases. With the international development of acupuncture, many scholars abroad have started to try to use acupuncture to treat premature ejaculation, and there have been many reports. According to the theory of traditional Chinese medicine, I created the method of tuning and fixing the essence of acupuncture, together with the treatment of western medicine, using a combination of acupuncture and medicine in the early stage and acupuncture alone in the later stage, with reliable efficacy, and specific The scientific research data is in the process of collation, looking forward to good results!
