What is cough variant asthma?

Cough variant asthma, from its name, we can see that it is first of all asthma, but it is not the same as typical asthma, but a special type of asthma, patients usually do not have the common wheezing, shortness of breath, croup in the throat and other such typical asthma manifestations, cough is its only or main clinical manifestation, so it is called cough variant asthma. It is characterized clinically by irritating dry cough, itchy throat, and severe coughing at night, which can be triggered or aggravated by cold air, oil smoke, dust, and haze. Its pathogenesis is similar to that of typical asthma in that it is caused by allergic reactions, hyperreactivity and chronic inflammation of the airways due to environmental, genetic factors and immune function, which leads to recurrent cough attacks. It is for this reason that about 30%-70% of patients with cough variant asthma in the clinic eventually develop into typical asthma due to untimely treatment and become lifelong diseases that can only be controlled but not cured, which seriously affects the quality of life, so we should pay high attention to this disease of cough variant asthma. Diagnosis of cough variant asthma relies on typical cough symptoms, as mentioned earlier: irritating dry cough, itchy throat, obvious at night, aggravated or triggered by cold air, oil and smoke, combined with the patient’s history of allergic rhinitis, pharyngitis, dermatitis or family history of allergy, and pulmonary function tests (bronchial excitation test, peak flow rate monitoring) for diagnosis. Modern medicine requires anti-asthmatic treatment for diagnosed cough variant asthma, including glucocorticoids and antispasmodic and wheezing drugs combined with inhalation, commonly used drugs include sulforaphane, Symbicort and nebulized inhalation therapy, oral hormones can be used if the effect is poor, and the leukotriene receptor antagonist montelukast can also be combined to improve cough symptoms and airway inflammation. Chinese medicine classifies cough variant asthma as “wind cough” because of its characteristics: itchy throat, dry cough, sudden onset and cessation, and the urge to cough when one speaks, which are consistent with the characteristics of “wind evil” as considered by Chinese medicine. It is believed that the pathogenesis of this disease is that the wind evil offends the lung and the lung loses its propagation and descent. Treatment is based on the principles of draining the wind and promoting the lung, stopping coughing and relieving pharynx. Chinese herbal medicine can be taken in the form of Huanglong Cough Capsules/Granules, which are more effective. For patients with severe and intense cough, a combination of Chinese and Western medicine can be used, which can enhance the efficacy and shorten the course of treatment. Cough variant asthma accounts for about one-third of chronic coughs, and more than one-third of the patients in the whistle clinic have chronic coughs, so cough variant asthma accounts for about 11% of the whistle clinic, which shows that the clinical incidence of cough variant asthma is still very high. Therefore, it is important to strengthen protection and prevent the disease before it happens. In terms of Chinese medicine theory, the occurrence of external diseases is considered to be the result of the inability of the good to overcome the evil, so the prevention of diseases should also be carried out from both the “good” and “evil” aspects. The first and most important thing is to improve the body’s positive energy, as early as 2000 years ago, the “Yellow Emperor’s Classic of Internal Medicine” tells us that “positive energy within, evil can not dry”, that is to say, the body’s positive energy is sufficient, evil will not invade, so we should try to improve the positive energy through the following measures. First, strengthen exercise, the right amount of exercise can enhance physical fitness; second, moderate supplementation, especially in winter, the most suitable for tonic, and the most effective way to winter tonic is to take oral creams, by taking oral creams to strengthen the spleen and benefit the lungs can improve the ability of the lungs to resist external evil, reduce allergic reactions; third, stimulate Yang Qi, can be in the three volcano days, three nine days acupuncture point paste to support the essence of Chinese medicine, to stimulate Yang Qi, improve The role of positive qi. Secondly, we should guard against the invasion of evil qi. For cough variant asthma, because it is an allergic disease, we must first avoid contact with allergens. Patients of this kind must carefully observe the causes and environmental conditions that trigger coughing each time, especially children, and parents must carefully observe and find allergens early; at the same time, some patients are more sensitive to cold air, wind and air conditioning, so they should avoid being cold and windy to reduce their exposure to evil qi. By doing these two things, improving positive energy and avoiding evil energy, we can effectively prevent cough variant asthma. At the same time, we also hope that we will pay attention to and be alert to cough variant asthma, and once recurrent cough occurs, go to a regular hospital for early treatment and early Chinese herbal intervention to avoid progression to bronchial asthma.
