Colorectal cancer is a malignant tumor that occurs in the colon, rectum and cecum. In Chinese medicine, the disease is classified as intestinal tumor, accumulation, dirty poison, locking anal hemorrhoids, etc., and has been recognized as an incurable disease. As stated in the book “Surgery (Dacheng)”: “Locked anal hemorrhoids are like bamboo joints locked inside and outside the anus, shaped like jellyfish. There is no cure for this.” However, early surgical radical treatment, postoperative chemotherapy and radiation therapy, combined with proper TCM evidence-based treatment, play a pivotal role in improving the quality of life of postoperative colorectal cancer patients, prolonging the survival period and increasing the survival rate. It should not be considered that the damage of cancer tumor to human body disappears after local removal of cancer tumor, but must be seen that the damage of cancer tumor to human body still exists in different degrees and has become a serious problem due to the serious damage of surgery to human body and the damage of radiotherapy and chemotherapy drugs to human body. Therefore, the integrated treatment of Chinese and Western medicine and psychotherapy is the current development direction for the treatment of this disease. The exact cause of colorectal cancer is still unclear, but its occurrence may be related to chronic inflammatory stimulation, malignant transformation of benign tumor, improper diet, ingestion of carcinogenic substances and genetic immune malfunction. According to traditional Chinese medicine, the disease is mostly caused by poor diet, internal dampness and heat, or by depression and depression, injury caused by the seven emotions, disharmony of the stomach and intestines, and Qi stagnation and blood stasis; or by prolonged diarrhea and prolonged dysentery, spleen deficiency and loss of transportation, internal growth of dampness and toxicity, and decrease of turbidity in the internal organs; or by the accumulation of cold and heat, phlegm and dampness, Qi stagnation and blood stasis, which accumulate in the anus and become this disease. As stated in the “Ling Shu? Nine Needle Theory says: “The four seasons and eight winds of the guest in the meridians, for the tumor disease also.” In the chapter on the true evil of the pricking section, it is also said: “The entry of the deficient evil into the body is also deep, cold and heat fight each other, stay for a long time and internal traces, the evil qi resides between them and does not revert, and develops in the tendon slip… intestinal slip.” However, surgery is only a local treatment, the other invasion of toxic evil and the accumulation of pathological products, as well as the injury of surgery radiotherapy chemotherapy, there may be a variety of complications, therefore, there is a difference between the standard and the urgent positive and the deficient. 2.1 Support the root of the tumor. Tumor is a visible thing and its property is Yin, which is called “Yang transforms Qi, Yin forms” in Nei Jing. The formation of tumor is often related to the deficiency of Yang Qi, cold and stagnation, such as “Ling Shu? The first of all illnesses is the cloud: “The first of all illnesses is the cold, and the tumor becomes a tumor.” Another example is “Longue Yue Quanshu? accumulation” and cloud: “Where the spleen and kidney deficiency and weakness of the disorder of the people have more accumulation of disease. Gai spleen deficiency is not transported in the middle jiao, kidney deficiency is not transformed in the lower jiao, the right qi can not be evil stagnation to live in.” Insufficient innate endowment, or deficiency of yang due to improper regulation, or depletion of qi and blood due to artificial injury from surgery or stimulation from radiotherapy can move the root. After years of clinical observation and research on postoperative colorectal cancer, three types of deficiency, namely Qi and Blood, Liver and Kidney Yin deficiency, and Spleen and Kidney Yang deficiency, are common. In the treatment, the treatment is to benefit the qi and strengthen the spleen, nourish the liver and kidney, and warm the spleen and kidney as the main method to support the deficiency, which can enhance the body’s immune function and improve the body’s ability to resist tumor so as to control the development of tumor. 2.2 Removal of evil and treatment of its symptoms The origin of tumor is certainly in the righteousness, but the formation of tumor must have a series of pathological changes, such as the accumulation of evil toxins, stagnation of Qi, stasis of blood, and coagulation of phlegm and dampness, which means that there is a symptomatic side. In terms of postoperative colorectal cancer, evil toxins, dampness and heat, stagnation of qi and blood are the real aspects of the pathological changes, and if these toxins and pathological products stay for a long time, the rapid changes of the disease may lead to many critical symptoms, such as rumbling, convulsions, convulsions, hemoptysis or even death of yin and death of yang, affecting the safety of others. In the treatment, according to the principle of “treating the symptoms when it is urgent”, the urgency should be slowed down first. Of course, in the course of treatment, if more attention is paid to the elimination of pathological toxins, it is often possible to avoid the emergence of critical symptoms. Through clinical observation, the damp-heat and toxin-embedded type and the qi-blood stagnation type are the most common. Damp-heat poisonous type: clinically, it is common to have loss of appetite, abdominal distension and pain, loose stools or shortness of breath, mucus and blood stools, pus in the stool, foul smell, ulcers that do not close for a long time, and foul smell of oozing material. After the operation, the cancer has been removed, but the whole body is still weak, poor, frequent and loose stools, vague pain in the abdomen, yellow face, red tongue, yellow greasy moss, slippery pulse; treatment should be to clear heat and detoxify dampness. Commonly used in the treatment of this disorder are: Baitou, Amaranthus, Stachybotrys, Phyllostachys, Semen, Mao Cixi mushroom, Phyllostachys, Phyllostachys, Red vine, Sander root, Fishy grass, etc. Qi and blood stagnation type, clinically seen: dullness and abdominal distension, intestinal pain, abdominal lump refusing to press, severe postoperative wound pain, pus and blood mucus in the stool, shortness of breath, purple and dull tongue with petechiae, astringent pulse. Treatment should be to move Qi, activate blood circulation and remove blood stasis. Commonly used drugs include Curcuma longa, Trigonella, Boswellia, Myrrh, Safflower, Rhizoma Chuanxiong, Radix et Rhizoma, Salviae Miltiorrhizae, Radix Paeoniae Alba, Citrus Aurantium, Houpao, Paeoniae. 2.3 Treating both the symptoms and the root cause of colorectal cancer is to treat the symptoms of various postoperative complications such as bleeding, infection and pain. However, cancer often manifests complex symptoms of deficiency and symptoms, so the treatment should take into account both the symptoms and the root; when the symptoms are urgent, the symptoms should be treated first, and when the root is the main cause, it is appropriate to consolidate its true yin and true yang and treat the symptoms at the same time. Both the symptoms and the root cause are commonly used in clinical practice. 3 Case example The patient is male, 82 years old. He was 5 days postoperative for rectal cancer Miles, complicated by intestinal obstruction, scrotal edema, deep vein thrombosis of the right lower limb, and slightly high skin temperature of the right lower limb. The patient had a preoperative combination of multiple cystic bronchiectasis in the left lower lung and infection. The patient was treated with intravenous drip of salvia injection and nasal feeding of enteric aspirin, combined with gastrointestinal decompression, maintenance of water-electrolyte balance, anti-infection and sputum treatment, etc. After 3 d, the patient defecated manually, but the scrotal edema and swelling of the right lower limb did not decrease. The patient was bedridden, thin, fatigued, drowsy, cold, coughing, white sputum, easy to cough up, no fever, sweating, preferring hot drinks; light red tongue, white slightly greasy coating, sinking pulse. The scrotum was enlarged, with pressure pain, bright surface, size about 10 cm×7 cm, no suppuration was seen. Ultrasound of the deep veins of the lower extremities showed that the right lower extremity had deep vein thrombosis and lumen obstruction, and the right inguinal lymph nodes were enlarged. The diagnosis of Chinese medicine is deficiency of kidney yang and cold, and internal obstruction of phlegm; treatment: warming yang, resolving phlegm, promoting lung and relieving cough. The formula is as follows: Radix et Rhizoma rehmanniae 10 g, 6 g of roasted ephedra, 10 g of dried ginger, 5 g of raw semen, 5 g of fine spice, 20 g of poria, 1.5 g of cinnamon, 10 g of fried Phellodendron, 15 g of fried Zhi Mu, 15 g of fried Atractylodes, 15 g of northern almonds, 15 g of orris, 15 g of dried eucommia, 10 g of roasted licorice. take one dose daily with water. The patient’s scrotal edema and swelling of the right lower extremity were significantly reduced after 3 days of taking the medicine, and the general symptoms improved significantly. The case was critically ill, and the intestinal obstruction, scrotal edema and deep vein thrombosis of the right lower extremity were not reduced after symptomatic treatment by western medicine. The patient’s fear of cold, preference for hot drinks, white phlegm, fatigue, and spontaneous sweating are clinical manifestations of Yang Qi deficiency; then the patient’s fatigue, drowsiness, and sunken pulse are identified. Also, in Su Wen? Yin and Yang Ying Xiang Dazheng” points out: the treatment of disease must seek its origin. The “essence” is yin and yang. For deficiency-cold diseases, Luo followed the principle of “treating cold with heat and warming the laborer” in the Nei Jing, so he identified this as Shao-Yin-Yang deficiency and internal obstruction of phlegm, and gave Zen Wu Tang plus reduction. The formula uses Radix et Rhizoma Pseudostellariae, Radix Ginger, Radix Sinensis and Cinnamon to warm Yang, Ephedra, Almond, Radix Platycodon and Radix Eucommiae to promote the lung to stop cough and resolve phlegm, Atractylodes Macrocephala, Poria and Radix Panax notoginseng to strengthen the spleen and dry dampness, and Radix et Rhizoma Glycyrrhizae, Fried Phellodendron and Zhi Mu to prevent the harm to Yin from the pungent and warm medicines in the formula. The whole formula is simple and powerful, effective as a rafter.
