Comprehensive treatment of polytrauma

Nutrition is the basis for biological growth and survival, and is the substrate for patients to resist external aggression, maintain physiological functions, repair tissues, and restore health. The purpose of surgical nutritional support is to reduce stress, avoid loss of lean body tissue, mitigate catabolism caused by fasting and starvation, support anabolism, and maintain normal organ function, thus promoting wound healing and disease recovery.
At present, the implementation of nutritional therapy should master the concept of comprehensive nutrition, and the two pathways of parenteral nutrition (PN) and enteral nutrition (EN) are complementary and mutually reinforcing. When both PN and EN are available, EN is preferred and supplemented with PN when EN cannot meet the requirements; PN is available for short-term improvement of nutrition, and EN should be applied by all means for long-term nutritional support; calorie supply advocates “low calorie supply”, the use of mixed energy and all-in-one continuous input, and the widespread use of three-liter bags to ensure the effectiveness of nutritional therapy.
Treatment of intestinal fistula biliary fistula caused by car accident: surgery + postoperative nutritional support (PN + EN)
imaging and drainage suggesting biliary-enteric fistula
          Various postoperative abdominal drains
Post-operative enteral plus enteral nutritional support
The patient was discharged from the hospital in good condition
           Steel bar penetration injury
The rebar entered from the perineum and protruded from the posterior back
X-rays showed the direction of the rebar in the body
The rebar was surgically removed and the patient was rescued
