Abstract: Vaginal bleeding is a phenomenon of vaginal bleeding, mostly caused by vaginitis infection. When vaginitis is not cured in time and the condition worsens with elevated routine blood leukocytes, it suggests the presence of vaginitis. The patient in this case came to the clinic with mild vulvar itching and increased leucorrhea 1 week ago, which gradually worsened, leucorrhea odor, and vaginal venting, which was clinically diagnosed as vaginal blowing. After giving antibiotic medication and vaginal douching, the condition was controlled and the abdominal pain and vaginal blowing disappeared.

I. Initial consultation
The patient reported that she had no history of sexual life before, and had sex with her boyfriend several times in the last month. 10 days ago, she had a cold and fever after accidentally getting wet, and her symptoms were relieved after taking medicine for 5 days. The patient thought she was infected with a sexually transmitted disease, but because she was unmarried and afraid of others, she did not dare to consult a doctor, which led to mental stress and poor sleep quality. Yesterday, she started to have abdominal pain and was persuaded by her friends to go to the hospital. Outpatient gynecological examination showed flushing of the vulva and vagina, yellow purulent vaginal discharge, painful lifting of the cervix and uterine pressure.
Treatment history
After admission, she was given routine blood tests, vaginal discharge examination and pelvic ultrasound. The results indicated that the routine blood leukocytes were elevated, trichomonas was found in the vaginal discharge, and the pelvic ultrasound showed no significant abnormalities. We explained to the patient that itching of the vulva, odor of the leucorrhea and the feeling of gas coming out of the vagina (pussy blowing) were caused by trichomonas vaginalis, which causes anaerobic fermentation of carbohydrates in the vagina, resulting in a putrid odor and the production of a lot of gas, which leads to the emission of gas from the vagina and the phenomenon of pussy blowing. The patient relaxed a lot after understanding her condition and was willing to cooperate with the treatment. She was given intravenous infusion of antibiotic drugs (Ceftriaxone sodium for injection + 0.9% sodium chloride injection and metronidazole injection) together with vaginal douche treatment with Gynecologic spray.
III. Treatment effect
After 3 days of drug treatment, the patient’s abdominal pain was relieved, vulvar itching was relieved, the number of episodes of pubic blowing was reduced, and the routine blood leukocytes decreased on recheck, indicating that the treatment was effective. After 7 days of treatment, the patient indicated that the abdominal pain was relieved, the vulvar itching and blowing disappeared, the routine blood and leukocyte secretion examination were normal, the vulva and vagina were not flushed on gynecological examination, the leukocyte was not excessive, the cervix was not painful, and the uterus was not painful on pressure, so the patient was discharged with medicine and was recommended to be reexamined at the outpatient clinic after 1 week. There is no tendency of recurrence and the gynecological examination is in good condition.
IV. Notes
We are glad that the patient’s symptoms have been relieved after treatment and there is no recurrence, but we suggest that the patient should still pay attention to the following matters after discharge.
1. Avoid sexual life during the treatment period, otherwise it will affect the treatment effect and easily lead to reinfection. And pay attention to perineal hygiene in life and change underwear regularly.
2, it is recommended that patients improve their body immunity, rest more, develop good habits, relax, ensure sleep, and carry out appropriate physical exercise in general.
3.Patients are recommended to eat light and nutritious food, more fresh vegetables, fruits, eggs, lean meat, etc. Try not to eat spicy and stimulating food.
V. Personal insights
The symptoms of pubic blowing in young women are often caused by trichomonas infection-based vaginitis, in which trichomonas infection produces putrid gas by anaerobically fermenting carbohydrates in the vagina. Trichomoniasis is mainly transmitted directly through sexual intercourse, so lack of hygiene, too frequent sex, and reduced immunity of the female body can easily cause the onset of the disease. However, delayed treatment can lead to a series of gynecological diseases, such as endometritis, pelvic inflammatory disease and infertility, which can have a negative impact on the body and mind of young women.
