Stroke hemiplegia rehabilitation should be gradual

  Limb spasms and sequelae can be prevented or reduced through postural placement and passive exercise after the onset of stroke. When the patient is conscious and the vital signs are stable for about a week, active rehabilitation exercises can be started.  1.Right and left turning in bed, which is one of the most basic trunk function training.     2.Patients practice sitting up from the healthy side or the affected side. Sitting is one of the easiest movements for the patient to accomplish, and is also necessary for the prevention of postural hypotension and some activities of daily living such as standing and walking in the future. It should be carried out simultaneously with the turning training, taking care not to let the patient sit with his back against the object.  3.Standing exercise should be started after the patient can sit independently. After the patient can stand alone, let the patient gradually shift the weight to the affected leg and train the weight-holding ability of the affected leg.  4.When the patient can stand independently and the affected leg can hold more than 75% of the body weight and can step forward, only then can the walking exercise be started. For most patients, it is not advisable to use a cane too early to avoid affecting the exercise of the affected side. Before walking exercise, practice alternate backward and forward steps and weight transfer of both legs.  5. Start occupational therapy after the patient is able to sit independently, which includes daily living ability training, such as eating, personal hygiene, dressing, bathing, doing housework, and participating in craft activities.  In addition, physical therapy and acupuncture are also performed, as functional electrical stimulation, biofeedback and acupuncture are helpful in increasing sensory input and promoting functional recovery and motor control.  Because of the differences in the severity of the patient’s condition, the patient’s cognitive ability and receptivity, rehabilitation exercises are best carried out under the guidance of a physician.
