1. Newly diagnosed idiopathic generalized epilepsy Valproic acid is the first-line drug and the only drug of choice for three seizure types (generalized tonic clonic seizures, atonic seizures, and myoclonic seizures). In addition to valproic acid, topiramate and lamotrigine are the first-line drugs for generalized tonic clonic seizures. When valproic acid fails, the first-line drug of choice for generalized tonic clonic seizures is lamotrigine, and the first-line drugs are lamotrigine, topiramate, and levetiracetam. Lamotrigine is the first choice and the first-line drug for anhedonic seizures. There is no drug of choice for myoclonic seizures, and the first-line drug is levetiracetam. The drug of choice for all 3 generalized seizures after failure of lamotrigine or topiramate treatment is valproic acid. 2. Newly diagnosed symptomatic partial epilepsy The initial drug of choice is both carbamazepine and oxcarbazepine. The first-line drugs were carbamazepine, oxcarbazepine, lamotrigine, topiramate, and levetiracetam, and in partial secondary generalized seizures, valproic acid entered the first-line drugs in addition to the above drugs. Carbamazepine, oxcarbazepine and lamotrigine are the drugs of choice after failure of other medications. Lamotrigine is the drug of choice after failure of carbamazepine or oxcarbazepine treatment. Valproic acid remains the first-line drug of choice as a 2nd option in patients with partial secondary generalized seizures. 3. Combination medication In the pharmacological treatment of idiopathic generalized epilepsy, valproic acid is the drug of choice in combination with other drugs. In the pharmacological treatment of symptomatic partial epilepsy, carbamazepine (oxcarbazepine) + topiramate, carbamazepine (oxcarbazepine) + levetiracetam, carbamazepine (oxcarbazepine) + valproic acid, valproic acid + lamotrigine, lamotrigine + carbamazepine (oxcarbazepine) and phenytoin + topiramate are various preferred combinations for the combined treatment of symptomatic partial epilepsy.
