Male breast enlargement, also known as gynecomastia, is a common condition that can occur at any age, mostly in middle and older age. It is more common unilaterally. The main manifestations are unilateral or bilateral breast swelling and pain, breast development, and disc-shaped tough nodules under the areola, some of which are similar in appearance to those of adolescent girls. The reason for this is that there are primary and secondary gynecomastia. 1, primary male breast enlargement: is caused by endocrine physiological disorders, mostly seen in adolescent sexual maturity, when the level of sex hormones change rapidly, producing a transient imbalance in the ratio of estrogen and androgen, or increased sensitivity of breast tissue to estrogen, causing breast development. In older men, due to testicular atrophy or hypofunction, the relative lack of androgens and the dominant effect of estrogen on the mammary glands can also cause breast enlargement. 2, secondary male breast enlargement: is due to a disease leading to endocrine dysfunction, caused by breast enlargement. The main diseases are testicular insufficiency or testicular inflammation, trauma, tumors, etc. resulting in testicular insufficiency, causing mammary hypertrophy. Male breast enlargement and breast hypertrophy have long been treated mainly for their etiology, which revolves around the imbalance of estrogen and androgen in the body. When conservative treatment is ineffective and surgical treatment is carried out, due to the short diameter of the nipple areola in men, the surgical removal of mammary tissue alone generally leaves a more obvious scar, and many patients, due to the thicker surrounding fatty tissue after mammary tissue removal, have a similar disc-shaped depression deformity on both sides of the chest breast area after surgery, i.e., depression at the nipple and areola. And the surrounding bulge, which seriously affects the aesthetics. With the development of fat aspiration, and many patients with gynecomastia, who simply underwent breast liposuction, only the fat around the areola was sucked out due to the surgery while the enlarged glands were not removed still remained. After surgery, the breast area shows a mound-shaped bulge, which also affects the aesthetics.
