Polycystic ovary syndrome

  Polycystic ovary syndrome (PCOS) is one of the most common causes of menstrual disorders in fertile women and is a major cause of female infertility. The main clinical manifestations are: menstrual disorders The main symptom of PCOS patients, often manifested as 5 or sporadic menstruation, amenorrhea is mostly secondary (refers to the cessation of menstruation for 6 months after the establishment of normal menstruation, or the cessation of menstruation for more than 3 cycles according to their original menstrual cycle), amenorrhea is often preceded by sporadic or excessive menstruation. There are also a few patients who show excessive menstruation or irregular bleeding. Hirsutism, acne The manifestation is abundant body hair, especially pubic hair, and the distribution is often male shaped (rhombus).  Obesity More than 50% of patients show obesity (body mass index ≥ 25), and their fat distribution and body posture are not specific.  Acanthosis nigricans Gray-brown pigmentation of the skin on the labia, back of the neck, axillae, under the breasts and groin, symmetrical, with thickened skin and a soft texture.  Ancillary tests include: basal body temperature measurement, B-mode ultrasonography, diagnostic curettage, laparoscopy, and hormonal endocrine measurement.  Diagnostic criteria: In 2003, experts from the American Society for Reproductive Medicine (ASRM) and the European Society for Human Reproductive Medicine (ESHRE) jointly proposed the diagnostic criteria for polycystic ovary syndrome, known as the “Rotterdam criteria”: two of the following three criteria can be established. (1) low or no ovulation; (2) clinical or blood measurements of hyperandrogenism; (3) ultrasound images of polycystic ovaries.  Treatment: General treatment (mainly for obese patients) includes exercise, diet control, and metabolism-lowering diet pills to reduce body weight, which may help restore ovulation and reproductive function. Medications are mainly used to improve and correct hormonal endocrine abnormalities. Surgical treatments such as ovarian perforation and ovarian wedge resection are also available. The vast majority of patients can be treated to ovulate and conceive.
