Problems and countermeasures of pelvic fracture treatment in China

  While surgical treatment of pelvic fractures is relatively simple, emergency care is very important. Because it is most important to save the patient’s life in the emergency stage. However, the current level of emergency treatment for pelvic fractures in China is relatively backward, and even though the patient’s life is saved, many problems are left behind. For example, how to talk about surgical reconstruction of pelvic fracture in the case of a bladder or colostomy? Therefore, the principle of emergency treatment of pelvic fractures should be to restore the normal bony structure of the pelvis as early as possible with the safety of the emergency patient. This facilitates the repair of the pelvic organs and creates the basic conditions for the functional recovery of the patient’s motor system in the late stage.  Our country started research in pelvic fractures for 30 years, but there are many deficiencies and still no unique treatment system has been formed.  First, the rapid emergency transport system is not yet widespread in China. Pelvic fractures are high-energy injuries, and most of these patients occur at the primary level. Due to the transport problem, most patients are treated at the primary level, and it is impossible to have a relatively large number of concentrated patients, but the experience is greatly limited. It can be predicted that this situation will continue in the coming years.  Second, the emergency structure is not set up consistently from hospital to hospital. Some hospitals have emergency centers led by major surgery, while some hospitals have emergency centers led by orthopedics. The division of labor in the discipline is too fine, resulting in the inability to cooperate well and form a unanimous opinion in the emergency process.  Thirdly, the current post-graduation continuing education system in China is not sound, and the traditional treatment methods for pelvic fractures are still used in primary hospitals. Meanwhile, there is no more recognized treatment standard in China at present, so the treatment of pelvic fracture is rather arbitrary and lack of summary.  In view of the above, we must establish several pelvic fracture treatment centers in China as soon as possible through multidisciplinary cooperation of academic organizations, and draw on the experience of various families in order to establish the treatment process of pelvic fracture emergency and the method of late surgical functional reconstruction, so that this technology can be popularized in a very short time and the emergency and treatment of pelvic fracture in China can reach the international level as soon as possible.
