Recently in the clinic, we saw a lively and lovely 10-year-old boy, Xiao Ming, whose mother cried and told us that a week ago when Xiao Ming was playing with his buddies with a laser pointer, his right eye was accidentally irradiated by his peers for a few seconds, and when he came home, he said he could not see well in his right eye and there was a white shadow in the center, the parents thought it would recover in a few days, but it never got better, so he came to the clinic. We examined the child and found that the best corrected visual acuity was only 0.4 in the right eye and 1.2 in the left eye. Fundus examination revealed that the reflection of the central macular recess in the right eye disappeared and the local pigmentation changed. Further OCT examination (figure below) revealed that the outer layer of the retina, mainly the IS/OS layer of photoreceptor cells and the RPE layer of the central macular recess, had a structure with a clear boundary missing (arrow). The macula is the most important structure of vision, the division of photoreception, form and color vision, and once damaged, it is very capable of recovering its function. We are very sorry to tell the crying mother that this is a macular burn caused by the laser pointer, which is difficult to recover and has caused permanent visual impairment in Xiaoming’s right eye, with the possibility of future complications, such as macular fissure, leading to further vision loss. What can be done now is to make sure to pay attention to eye hygiene, protect the residual vision in the right eye, and even more so, protect the left eye and review it regularly. Both Xiao Ming and his mother did not expect that a small laser pointer would cause such serious irreversible damage. Because of the laser pointer’s strong focus, the irradiation site can produce a heat effect, to meet the psychology of minors fresh hunting, and thus widely welcomed by primary and secondary school students. (Roadside vendors peddling laser pointer, the buyer to the middle of the road waving laser pointer, affecting traffic safety, the vendor said: “high-powered laser pointer is very bright, irradiated against the camera lens are able to burn imaging components.”) At present, China’s current GB7247 standard laser on the human body is divided into four levels of risk, measurement standards for the observation of laser within the beam of the MPE (maximum possible impact) on the eyes: primary and secondary (0.4 to 1 milliwatt), as long as the exposure time is not long, the retina and skin generally will not cause harm; but more than 3 to 5 milliwatts of laser pointer power is larger, can cause human injury. Due to the lack of regulation, some of the laser pointer sold around the campus power up to 100 to 300 milliwatts, can instantly ignite matches, firecrackers and paper, and even start a fire. In addition to the proliferation of network sales, because of the lack of network supervision, many primary and secondary school students can easily buy high-powered “laser pointer” from the Internet. In addition, primary and secondary school students lack the necessary self-protection awareness, play, a little longer exposure, can cause damage to the eyes, skin surface, resulting in permanent damage, or even blindness. In recent years, we have seen several cases of children like Xiao Ming in our outpatient clinics. Some children can clearly say when they have been exposed to the laser pointer, while others do not remember, but only admit to having played with the laser pointer recently when we ask them. In the face of such children, we are saddened and call on parents to educate their children to stay away from such laser products, not to look directly at the light source out of curiosity, and not to irradiate others, so as not to cause irreversible consequences. Of course, as doctors, we hope that the state can introduce appropriate policies to strictly monitor the production and sale of laser pointer, to protect our next generation.
