Dysfunctional uterine bleeding refers to irregular vaginal bleeding that does not occur during menstruation, also known as dysfunctional uterine bleeding, or “gonorrhea” for short. It is also called “dysfunctional uterine bleeding”. Clinically, prolonged menstrual periods of more than two weeks are also classified as gongbao. Dysfunctional uterine bleeding belongs to the category of “collapse and leakage” in Chinese medicine. It is called “collapse” when there is more than one big drop, and “leakage” when there is a drop. According to Chinese medicine, the main cause and mechanism of the disease is kidney deficiency and loss of storage. Or the deficiency of kidney yin, deficiency of yin and loss of guardianship, and delusional movement of deficiency fire and bleeding. Secondly, spleen deficiency and Qi trapping, lack of power to control and regulate, loss of consolidation of the ramifications, and loss of restraint of essence and blood. Thirdly, internal heat of the blood disturbs the sea of blood, forcing the blood to move delicately, and the rushing of the blood is out of order. In addition, stasis of blood blocking the ramifications also causes the blood not to return to the menstrual flow, resulting in collapse and leakage. In conclusion, the etiology of this disease is complex, the origin of the disease is in the kidney, the position is in the ramus, and the change is in the qi and blood, which is manifested by the blood of the uterus going down at different times, therefore, the qi and blood are often involved in the same disease, many organs are involved, the deficiency is mixed with the reality, the heat is more than the cold, the cause and effect are related, and the situation is repeated, which can be transformed into deficiency of qi and blood, or both qi and yin, or both yin and yang. Therefore, if clinical symptoms of leakage occur, do not take any chances, but go to the hospital for examination and treatment as soon as possible, so as to shorten the course of treatment and not to involve other organs and produce many adverse consequences. For the treatment of collapse, we should flexibly apply the methods of “plugging the flow, clarifying the source and restoring the old”, or use the same method of plugging the flow and clarifying the source, or clarifying the source and restoring the old at the same time. (1) Clearing and stopping is used for the heat evidence of bleeding. The symptoms include heavy bleeding and bright red blood color. The heat symptom of bleeding is often related to the deficiency of liver and kidney yin and hyperactive phase fire, which disturb the sea of blood. It is advisable to use clear and nourishing products, such as peony bark, raw earth, white wormwood, ground elm, and so on. As for the bitter cold descending diarrhea products such as Huang Lian, Huang Bai, Gardenia, etc. should be used with caution to avoid bitter cold injury to yin. (2) Warming and stopping is used for those whose bleeding belongs to deficiency cold. The symptoms include small amount of bleeding, light color, and cold pain in the abdomen. It is not advisable to use pungent, slippery, dry and hot products, but to use deer horn gum, bacopa monniera, dog’s back, cuscuta, and ginseng and astragalus to warm the yang and benefit the qi, and to replenish the fire in water. If the cold is more serious, can be added at the discretion of the dogwood, ginger, aconite, aromatic herbs and other warm managers to stop the blood. (3) Tonifying and stopping is used in cases where menorrhagia is caused by deregulation of Qi and blood in the liver, kidney, spleen, stomach, three yin and three yang, weakened function, and loss of punching power. Clinical manifestations include irregular and incomplete menstrual bleeding, accompanied by fatigue, dreaminess and poor appetite. Generally, liver and kidney deficiency or spleen and kidney deficiency are the most common symptoms. The treatment of liver and kidney deficiency is to nourish the liver and kidneys and harmonize yin and yang, with medicines such as chasteberry, dried lotus grass, parasite, and dulcimer. In the two deficiencies of the spleen and kidney type, not really in the lower qi, clear Yang does not lift the evidence, generally should not use Shengma, Chai Hu and so on, so as not to shake its root plant, move its floating Yang, in order to speed up its disaster. (4) Stopping by diarrhea, used in cases of Qi stagnation and blood stasis of collapse. The symptoms include distension and pain in the chest and abdomen, with dark blood and blood clots. The treatment is to invigorate the blood and remove blood stasis, and to promote the generalization of the cause. Commonly used are Paeonia lactiflora, Radix et Rhizoma, Lactuca lactuca, Trigonella, Curcuma longa and Panax ginseng. In the bleeding period of other syndromes, appropriate products should also be used to invigorate blood circulation and resolve blood stasis in order to achieve the function of resolving blood stasis and generating new blood. Otherwise, stagnation will occur if the remedy is not accompanied by circulation, stasis will occur if the astringent is not accompanied by circulation, and condensation will occur if the clear is not accompanied by circulation. For the identification of bleeding, attention should be paid to the presence or absence of abdominal distension, abdominal pain and the nature of distension and pain, as well as to the shade of blood, the consistency of blood and the presence or absence of blood clots. We should also observe the color of the blood, the consistency of the blood and the absence of blood clots. We should also refer to the tongue, pulse and systemic symptoms to distinguish between heat and cold and deficiency. The main purpose of typology is to facilitate treatment, but it should not be limited by the types, because it is sometimes difficult to separate them clinically, and they are often intertwined, and the types can be transformed into each other. In terms of diagnosis, we should adopt the method of combining disease identification and evidence identification. Since the causes of uterine bleeding are complicated, necessary clinical tests must be performed to clarify the diagnosis and then differentiate the type. For example, if the diagnosis in Western medicine is uterine tumor and the diagnosis in Chinese medicine is weakness of qi and blood, the treatment should not only nourish qi and blood to strengthen the root, but also adopt the method of activating blood circulation and eliminating blood stasis to eliminate Y accumulation, so as to improve the therapeutic effect. In the post-treatment of the leaking tumor, the role of liver, kidney, spleen and stomach, especially the spleen and kidney, should be emphasized. The kidney is the root of the innate nature and is the root of Qi and blood biochemistry; the spleen is the root of the postnatal nature and is the source of Qi and blood biochemistry; the liver is the master of drainage and is the driving force of blood flow. Therefore, according to the clinical situation, either the spleen and kidney should be supplemented together or the liver and kidney should be nourished. In general, attention should also be paid to the diet. Make a regular diet and keep a relaxed mood. Eat appropriate amounts of red beans, red dates, cinnamon, peanuts, black beans, black rice, black sesame and other foods that nourish the blood and benefit the kidneys to ensure that the kidney energy and liver blood are full and less likely to cause collapse.
