Generally speaking, auricular and external auditory microtia, as well as middle ear deformities, often occur simultaneously, so the surgery should address two aspects: 1) appearance: auricular reconstruction; 2) hearing improvement: external auditory tympanic reconstruction. In order to achieve a satisfactory appearance and improved hearing, while saving the number of surgeries, reducing the patient’s pain, and saving money, it is best to plan each phase of surgery according to the patient’s situation, such as hearing or appearance first? When to start the first surgery? How to reserve the position of the reconstructed ear canal opening? Small ear deformity is difficult to achieve the ideal result if the arrangement is not good and the position of the reconstructed ear and the ear canal opening are not coordinated. That is why the first treatment is critical and will affect the subsequent surgery. I. Unilateral deformity: Good hearing on the opposite side, mainly to solve the appearance problem, auricular reconstruction 1. Surgery time: after 6 years old, 1 year before school best after 30 years old must use artificial ear stent; 2. Surgery method: in 3 stages Phase 1: Takes about 2 months, only about 1 month in the hospital. Surgery: placement of dilators behind the stumped ear (hospitalization for about 1 week) Water injection: come to the hospital every 1 day after discharge (about 1 month) Skin maintenance: lasts for about 1 month or so, and the patient can go home. Phase 2: hospitalization for about 2 weeks. Surgery: Autologous rib cartilage scaffold implantation, auricular reconstruction Stage 3: Six months after stage 2 surgery, hospitalization for about 2 weeks days. Surgery: Reconstructive auricular revision. For those who want to improve their hearing, external auditory canal and tympanoplasty will be performed at the same time. Small ear deformity Bilateral small ear deformity: Hearing and appearance problems need to be solved at the same time. 1. Patients over 6 years of age: first perform the 1st and 2nd stage of surgery according to the 3-stage auricular reconstruction method, and then perform external canal and tympanoplasty or implant hearing devices in the 3rd stage. 2.Patients under 6 years old: wear hearing aids first. For congenital ear deformities that prevent the wearing of hearing aids, softband BAHA or sound bridge implantation can be used, and the auricle will be reconstructed after 6 years old.
