Swallowing Discomfort or Esophageal Cancer Alert

Esophageal cancer is one of the most common malignant tumors of the digestive tract, which is called “choking” in Chinese medicine. It is mostly seen in men over 40 years old, with peak incidence between 50 and 70 years old. Although the mortality rate of esophageal cancer is high, it can be treated well or even cured if it can be treated timely and appropriately. For early and middle stage esophageal cancer patients treated by our department, the efficacy of surgical treatment is satisfactory, and more than 80% of patients can survive for more than five years. Reviewing the esophageal cancer patients admitted to our department, almost all of them were poor common people when they were young. The causes of the disease are related to dietary habits (eating very hot food, drinking strong alcohol, eating salted fish, moldy food, etc.), malnutrition and lack of micronutrients (lack of vitamins, essential fatty acids and lack of micronutrients such as aluminum in the water source and soil, etc.), lack of nitrosamines, and related to diseases and hereditary factors. Symptoms of patients are mostly atypical, which mainly include: choking sensation of swallowing food, pain behind the sternum and under the raphe or dysphagia, slow passage of food or stagnation sensation, foreign body sensation in the esophagus, dryness and tightness in the throat, as well as symptoms of stuffy discomfort and belching behind the sternum in a small number of patients; when esophageal cancer develops to a certain degree, a series of typical symptoms related to swallowing disorders will appear, which are persistent and become more and more obvious , such as difficulty and pain in swallowing, spitting foamy mucous sputum, emaciation and dehydration, weakness and powerlessness. Patients often come to the doctor only at this stage, missing the best time for treatment, and only a small percentage of patients can receive surgical treatment. China is a country with high incidence of esophageal cancer and also has the highest mortality rate of esophageal cancer. At present, the patients we admit and treat are mainly from central Shandong and southwestern Shandong. Surgery-based comprehensive treatment is the development direction of esophageal cancer treatment, which has been affirmed by most scholars for its role in improving surgical resection rate and reducing local recurrence rate, and has brought about a great improvement in the therapeutic effect of middle- and late-stage esophageal cancer. The surgical resection rate of esophageal cancer in our hospital has reached more than 98%, and those who failed to be resected also underwent “rerouting” surgery, in which a case of “rerouting” patient underwent postoperative radiotherapy and has survived for 10 years with good quality of life. For some patients with lung metastasis and bone metastasis, we have adopted multidisciplinary treatment with chemotherapy, radiotherapy and traditional Chinese medicine, which have achieved encouraging curative effects. As the treatment of esophageal cancer progresses, a comprehensive treatment method mainly based on surgery is mostly favored. For esophageal cancer in upper, middle and lower esophagus, as long as surgical resection is possible, surgery should be performed as far as possible, and then other auxiliary comprehensive treatments should be given after surgery. If the patient’s body does not allow surgical resection, for example, if the patient is weak, physically exhausted or has other diseases that are not suitable for surgery, radiation therapy can be used.
